Comparativa entre la rehabilitacion postquirúrgica del ligamento cruzado anterior (LCA) en el entorno privado y público: estudio transversal mediante encuestas

Abstract

INTRODUCCIÓN: El ligamento cruzado anterior (LCA) es uno de los principales estabilizadores de rodilla. Su lesión se considera grave, ya que se suele recurrir a la reparación quirúrgica, y los procesos de rehabilitación y recuperación son largos. La rehabilitación según las guías de práctica clínica son la principal referencia para conseguir una vuelta a la vida previa a la lesión por parte de los pacientes. OBJETIVO: Realizar una comparativa del abordaje de una lesión de LCA entre la sanidad pública y privada. MATERIAL Y MÉTODOS: Estudio transversal analítico sobre el tipo de lesión de LCA y su trabajo posterior a través de una encuesta a nivel nacional. RESULTADOS: La sanidad pública presentó peores datos en todas las variables de estudio (tiempos de espera, valoración funcional y opinión del paciente) respecto a la sanidad privada. Por otra parte, los resultados muestran una correlación negativa entre tiempos de espera y la puntuación de los ítems de funcionalidad del paciente en ambas vías sanitarias. Aun así, la sanidad privada es la más estable en todos sus resultados cuando se relaciona con tiempos de espera. DISCUSIÓN/CONCLUSIÓN: Se observa que ambas sanidades funcionan de una forma deficiente de cara a lo descrito por los autores, aunque es la sanidad privada la que más se acerca a los estándares estudiados. Por otra parte, comprobamos que la hipótesis del estudio coincide con los resultados puesto que la sanidad privada presenta mayor calidad que la pública en lo que al proceso sanitario se refiere (operación tiempos de espera y rehabilitación).

INTRODUCTION: The anterior cruciate ligament (ACL) is one of the main stabilizer of the knee. Its injury is considered as severe, with a high sanitary cost due to normally recurring to surgical repairing and having long rehabilitation and recovery processes The performance of the rehabilitation following the practical clinic guides is the main reference to achieving a returning to previous life before the injury for the patients. OBJETCIVE: Performing a comparative of the approaching of an ACL injury between the public and private health systems. MATERIAL AND METHODS: Analytical cross-sectional study on the type of ACL injury and it’s posterior work through a national survey. RESULTS: The public health system presented worse data in all variables of study (waiting times, final evaluation, funcional value and patient’s opinion) in contrast with the private health system. Results also show a negative correlation between waiting times and punctuation of the funcionality items of the patien in both health systems. Even so, the private health system is the most stable of all its results when related to waiting times. DISCUSSION/CONCLUSION: Comparing the actual papers we observe that both health systems work in a deficient way according to what the authors say, although the private health system is the one that gets closer to the studied standards. Besides, we observe that the hypothesis of the study matches with the results since the private health system provides greater benefits and presents a better quality than the public ones regarding the sanitary process (operation waiting time and rehabilitation).
