Intervenció fisioterapèutica en persones grans hospitalitzades en unitat d’aguts i subaguts per disminuir els efectes del repòs perllongat i complicacions

Abstract

RESUM: INTRODUCCIÓ: La immobilitat prolongada en pacients grans hospitalitzats redueix la funcionalitat i augmenta el risc de fragilitat i complicacions. La fisioteràpia, mitjançant a mobilització precoç i l’exercici terapèutic, és clau per prevenir i gestionar aquests efectes adversos. OBJECTIUS: Analitzar l’efectivitat de la fisioteràpia en la millora funcional i la reducció de complicacions en pacients grans hospitalitzats en unitats d’aguts. RESULTATS: En l’àmbit d’agnòstic, la valoració funcional inicial a través de proves complementàries (Escales de fragilitat, test de força muscular, valoració de la mobilitat i test de funcionalitat i proves bioquímiques) i el seguiment periòdic són les eines amb major suport. Els resultats finals permeten classificar els pacients segons el seu grau de risc de patir complicacions associades al repòs perllongat. En quant als pacients amb risc baix, se’ls hi aplica un programa de mobilització precoç o exercicis adaptats. Als pacients amb risc moderat o alt se’ls hi proposa una intervenció fisioterapèutica intensiva i personalitzada per prevenir i tractar les complicacions detectades en aquell pacient. CONCLUSIONS: L’abordatge fisioterapèutic en pacients geriàtrics hospitalitzats és complex, i la fisioteràpia és essencial per prevenir complicacions i millorar la recuperació. ABSTRACT: INTRODUCTION: Prolonged immobility in older hospitalized patients reduces functionality and increases the risk of fragility and complications. Physiotherapy, through early mobilization and therapeutic exercise, is key to preventing and managing these adverse effects. OBJECTIVES: To analyze the effectiveness of physiotherapy in functional improvement and the reduction of complications in elderly patients hospitalized in acute care units. RESULTS: In the diagnostic setting, initial functional assessment through complementary tests (Frailty Scales, muscle strength testing, mobility assessment and functional testing, and biochemical testing) and periodic monitoring are tools with greater support. The results allow patients to be classified according to their risk of developing complications associated with prolonged bed rest. For patients at low risk, an early mobilization program or adapted exercises are good to apply. For patients at, moderate or high risk, an intensive and personalized physiotherapy intervention is proposed to prevent and treat the complications detected in each patient. CONCLUSIONS: The physiotherapeutic approach in hospitalized geriatric patients is complex, and physiotherapy is essential to prevent complications and improve recovery.
