Influència dels aspectes biopsicosocials en el resultat de la histerectomia: avaluació i aproximació terapèutica

Abstract

El propòsit del present treball és revisar la influència dels aspectes biopsicosocials en el resultat de la histerectomia per a conèixer quines complicacions i/o alteracions s’han de tenir en compte en el moment de fer un tractament personalitzat. D’aquesta manera, l’objectiu és fer una recerca bibliogràfica actual basada en l’evidència que pugui aportar informació sobre quins factors es veuen alterats des de un punt de vista tant físic, psíquic com social. La histerectomia és una intervenció quirúrgica practicada freqüentment però, tot i així, avui en dia encara es desconeixen moltes de les repercussions i complicacions que comporta. Al llarg del treball es tracten diferents temes. Les possibles complicacions fisiològiques causades per la histerectomia, les alteracions a llarg termini, les adherències i el dolor juntament amb la modificació dels estrògens i els canvis en l’activitat de la paret abdominal són alguns dels punts que es troben desenvolupats. Aquí, s’hi ha de sumar les influences psicològiques, sexuals i contextuals (entorn) que envolten a les pacients a qui se’ls practica aquest tipus de cirurgia. El conjunt d’aquests coneixements, permetrà al fisioterapeuta realitzar una avaluació i una aproximació terapèutica basades de forma més detallada en el model biopsicosocial en el context d’un tractament multidisciplinar d’aquest problema de salut. Després d’aquesta revisió pensem que, malgrat la manca d’evidències científiques actuals de la seva aplicació, aquest abordatge probablement és el més apropiat per tal d’obtenir un restabliment més complert i precoç i per tant més satisfactori per la dona que es sotmet a aquesta intervenció.

The purpose of this paper is to review the biopsychosocial aspects that may influence the outcome of hysterectomy to know what complications and/or changes must be taken into account when making a personalized treatment. Thus, the goal is to present a literature search based on evidence that can provide information on what factors are altered from the point of view both physical, mental and social. Hysterectomy is a surgical operation developed frequently, however, nowadays are still unknown some of the effects and complications that hysterectomy entails. There are various topics discussed throughout this work. The physiological complications caused by hysterectomy, long-term changes, adhesions and pain along with the modification of estrogen and changes in the activity of the abdominal wall are some of the points that are developed. Here, it must be added the psychological, sexual and environmental influences that surround the patients who were practicing this type of surgery. All these skills will allow the physiotherapists to evaluate and to develop a therapeutic approach in more detail based on the biopsychosocial model in the context of a multidisciplinary treatment of this health problem. After review, we believe that despite the current lack of scientific evidence of this application, this approach is probably more appropriate to obtain a more complete and precocious restoration and therefore more satisfying for the women who undergoes this intervention.
