Conflicto femoroacetabular anterior en la cadera del adulto: diagnóstico, valoración y tratamiento en fisioterapia

Abstract

El conflicto femoroacetabular es una disfunción mecánica que afecta a la articulación coxofemoral, con más frecuencia en una población atlética de mediana edad. Existen diferentes factores que pueden llevar al conflicto: internos, como las deformaciones anatómicas de la cadera o descentrage y externos, como microtraumatismos de repetición durante los movimientos de flexión, aducción y rotación interna o externa. Su diagnóstico en fisioterapia no tiene suficiente especificidad y el tratamiento de elección que se lleva a cabo actualmente es demasiado invasivo. Por ello, la propuesta en este trabajo de final de grado es analizar esta disfunción, su diagnóstico y examinar las intervenciones físicas basadas en la evidencia científica. A raíz de una exhaustiva revisión de los estudios actuales, se establecerá un protocolo de tratamiento lo más completo posible. Material y métodos: se ha llevado a cabo una búsqueda bibliográfica mediante las bases de datos de Pubmed, PEDro, Pubmed central y Elseiver hasta el año 2005 con las siguientes palabras clave: femoracetabular impingement, mechanical dysfunction hip, hip biomechanics, anatomy of the hip, hip mechanical dysfunction treatment, femoracetabular impingement diagnosis, hip pain, physiotherapy treatment.

Femoroacetabular impingement is a mechanical dysfunction that affects the hip joint, most generally to middle-aged athletes. There are some factors that can cause the impingement such as inner anatomical deformities of the hip and other disorders or external repetitive microtraumatisms caused by the flexion, adduction and/or internal rotation movements. Its physiotherapy diagnose is not specific enough and the corresponding treatment that is used nowadays is, almost, only surgical. Therefore, the purpose of this review of other reviews is to analyze this dysfunction, its diagnose and to examine the based on the evidence physical interventions. There is going to be introduced a complete treatment protocol, as a result of an exhaustive review of the current studies. Material and methods: a bibliographic research has been done using Pubmed, PEDro, Pubmed central, Elseiver databases dated until 2005 with the keywords: femoroacetabular hip impingement, mechanical dysfunction hip, hip biomechanics, anatomy of the hip, hip mechanical dysfunction treatment, femoroacetabular impingement diagnosis, hip pain, physiotherapy treatment.
