Lumbàlgies causades per ptosis renal en pacients adults i sense patologia orgànica associada: valoració i tractament de fisioteràpia

Abstract

La ptosi renal és una disfunció de la mobilitat del ronyó. El ronyó esdevé excessivament mòbil, les estructures de fixació fallen i descendeix de la seva posició anatòmica. En molts casos aquest descens es torna simptomàtic; provoca dolor, que sovint es tradueix en una lumbàlgia, i pot donar signes genitourinaris associats. Es descriuen 3 graus de ptosi segons la caiguda i les seves característiques, ja que impliquen estructures diferents. El plexe lumbar se situa en una posició retrorenal i una ptosi pot comprimir els nervis del plexe lumbar i donar dolor irradiat d’aquests; depenent del grau de ptosi pot comprimir-ne un o un altre. Pot donar també dolor lumbar per tensions miofascials com ara espasme o punts gatell del psoes i el quadrat lumbar, estructures en contacte directe amb el ronyó. Per abordar aquesta patologia es realitzarà una anamnesis inicial per esbrinar-ne la causa. Seguidament una exploració de la ptosi i el seu grau, valoració de les implicacions viscerals, músculesquelètiques i nervioses que comporta. Com a conseqüència es planteja un pla d’abordatge terapèutic de fisioteràpia a partir de teràpia manual visceral, tècniques sobre les estructures lumbars associades, regulació del sistema nerviós autònom, tècniques neurodinàmiques, tractament de la musculatura implicada, control motor de tronc i maluc i correcció postural. Aquesta guia d’actuació clínica està enfocada a influir en totes les estructures relacionades amb el ronyó per tal de que es recuperi la seva posició i funcionalitat.

Nephroptosis is a disfunction of the renal mobility. The kidney becomes too much moveable, there’s a failure of the fixing structures and it falls down from its anatomical position. In many cases this dropping will be symptomatic; it causes pain, normally a low back pain and it can yield genitourinary related signs. There are 3 degrees of ptosis described according to the dropping and its characteristics, being that it involves different structures. The lumbar plexus is situated in a retrorenal position and in a ptosis it may be compressed and result in irradiated pain; depending on the ptosis degree, it will affect a different nerve. It can also produce low back pain because of the miofascial tensions as a psoas or quadratus lumborum spasm or trigger point, which are in direct contact with the kidney. With the purpose of approaching this pathology, an initial anamnesis will be done in order to find out the cause of the problem. Then, there will be an assessment of the ptosis, its degree, the visceral, musculoskeletal and nervous implications. As a consequence, it arises physiotherapy therapeutic management plan beginning with visceral manual therapy, techniques on the lumbar structures associated, adjustment of the autonomous nervous system, neurodinamic techniques, treatment of the muscles involved, trunk and hip motor control and postural correction. This clinical practice guide is focused to affect to all the kidney related structures and it has the aim to recover its position and its functionality.
