Valoració i tractament manual de les disfuncions mecàniques de la pelvis adulta d’origen sacroilíac per inestabilitat

Abstract

En aquest treball es parlarà dels problemes mecànics que pot patir la pelvis adulta d’origen sacroilíac amb un component d’inestabilitat. Per centrar el tema i per tal de poder fer un diagnòstic específic es farà un petit recordatori anatòmic, seguit d’una part de biomecànica de la pelvis per entendre la funció d’aquest en tot l’aparell locomotor i com pot veure’s alterada, tot explicant les disfuncions mecàniques de la pelvis. Un cop clar el funcionament de la pelvis i les seves possibles alteracions s’ha fet una proposta de valoració no centrada en les disfuncions concretes sinó en les alteracions funcionals que es donen. L’objectiu del treball és veure com aquesta valoració ens porta al diagnòstic de la inestabilitat sacroilíaca i com es diferència clarament, a través d’aquesta exploració, d’altres possibles alteracions funcionals. Això porta a introduir una valoració molt exhaustiva de tot el complex lumbar-sacre-pelvis-maluc; per tal de definir amb claredat l’origen de l’alteració es fa necessari avaluar la mobilitat passiva, la mobilitat intraarticular (entre superfícies) i la capacitat de lliscament; serà així com s’obtingui una perspectiva de quin es l’estat de congruència articular i la integritat dels elements passius d’estabilitat. Posteriorment cal testar la capacitat funcional activa, per tal d’avaluar l’estabilitat activa que donarà una certa impressió del comportament dinàmic de la pelvis. Donat que el tema d’estudi es la inestabilitat de sacroilíaca el tractament es centra, per tant, en la estabilització activa i/o control motor de sacroilíaca.

This paper will discuss the mechanical problems that may suffer adult pelvis sacroiliac origin with component instability. To focus on the subject and in order to make a specific diagnosis, there will be a small anatomical and pelvic biomechanics reminder to understand its function throughout the musculoskeletal system and how it can be altered, even explaining mechanical dysfunction of the pelvis. Once the pelvis functions and possible alterations are clarified, there has been made a proposal assessment focused on possible functional disorders, instead of specific disorders. The purpose of the paper is to see how this evaluation leads to the diagnosis of sacroiliac instability and shows its difference from other potential functional alterations. This leads to introduce a thorough review of the entire complex lumbar - sacral - pelvic - hip; to clearly define the origin of the alteration is necessary to evaluate the passive mobility, mobility intra-articular (between surfaces) and the ability to slide; thereby we will also obtain a view of the status of articular congruence and the integrity of passive elements of stability. Thereupon, it should be tested the active functional capacity, in order to assess the active stability that will enable a perception of the dynamic behavior of the pelvis. Since the topic of study is the sacroiliac instability, the treatment focuses, therefore, on active stabilization and / or sacroiliac motor control.
