Tractament de la patologia discal lumbar en relació amb la sedestació perllongada

Abstract

Aquest text es tracta de una revisió sistemàtica en la que s’explica i demostra la influència de la sedestació perllongada sobre els canvis patològics del disc intervertebral. Aquest tipus d’afectació es pot veure en joves durant l’etapa estudiantil, ja que aquests passen una mitja de vuit hores asseguts i gairebé la meitat han reportat dolor lumbar alguna vegada. El disc intervertebral és capaç de provocar trastorns dolorosos a la columna lumbar quan es troba afectat. A part el disc també té una funció propioceptiva, per tant ja sigui pel dolor com per l’afectació de la funció propioceptiva, la patologia discal es troba íntimament relacionada amb la inestabilitat de columna. Tant l’avaluació com el tractament de la patologia discal es dividirà en dues parts. S’utilitza un tipus d’exploració per el segment lumbar en el que s’avaluen els diferents moviments de la columna i quina és la resposta al dolor i el grau de rigidesa del segment. A partir d’aquesta informació s’elabora un tractament manual basat en el comportament dels símptomes respecte a les diferents proves. Per aquest tractament és important saber quin tipus de patologia s’està tractant per conèixer les limitacions del pacient i els objectius del fisioterapeuta però les tècniques no es basen en el títol diagnòstic. La segona part consisteix en avaluar la funció de la musculatura estabilitzadora del segment afectat i tractar-la mitjançant exercicis de control motor.

This text is a systematic review which explains and proves the lengthen sitting posture influence on the pathological changes of the intervertebral disc. This type of affectation can be seen in youth during the high school period, because they spend an average of eight hours sitting and almost the half of them have reported low back pain once. The intervertebral disc is able to cause pain upsets in spine when is affected. Otherwise the intervertebral disc also has a propioceptive function, then the pathology of the disc is intimately related with the spine instability because of the pain and the affectation of the propioceptive function. The assessment and treatment of the intervertebral disc pathology will be divided in two parts. To evaluate the lumbar spine segment, the different movements of the lumbar spine will be assessed to determinate the pain response to them and the stiffness grade of the segment. Using this information the therapist can elaborate a manual therapy guide based on the symptoms behavior regarding the different tests during the assessment. It’s important to know which type of pathology is going to be treated to know the patient’s limitations and the therapist’s goals, but the treatment techniques can’t be based on the diagnosis title. The second part consists in assess the stabilizing muscles function of the affected segment and treat them using kinetic control exercises.
