Preservación, defensa y respeto de la dignidad humana en cuidados paliativos: revisión de la literatura

Abstract

INTRODUCCIÓN : La Dignidad Humana y los Cuidados Paliativos van insolublemente unidos cuando se afronta la toma de decisiones en el tratamiento de enfermos terminales. La dignidad en Cuidados Paliativos acaba siendo el principal y último objetivo de las intervenciones a llevar a cabo. Diferentes revisiones que se han llevado a cabo a nivel mundial ofrecen una visión de lo que está ocurriendo en nuestro país; se ha concluido por ejemplo que existe un vacío entre la investigación y la práctica de la dignidad en Cuidados Paliativos. Chochinov (2012) expresa que defender la dignidad en la atención sanitaria es como tener que defender que para vivir es necesario respirar. OBJETIVO : Revisión de la literatura sobre los Modelos Teóricos de la Dignidad Humana en la atención al final de la vida, y así dar respuesta al significado de dignidad, y cómo se preserva, defiende y respeta en Cuidados Paliativos. METODOLOGÍA : Se realizó una búsqueda bibliográfica entre los meses de Diciembre y Abril de 2014-2015 en: Pubmed, Google Académico y Scielo. Se seleccionaron todos aquellos artículos que se ajustaban a la pregunta de investigación y cumpliesen todos los criterios de inclusión. 3 RESULTADOS : Se identificaron 155 artículos, finalmente incluyendo un total de 32, entre ellos 12 ensayos. Los resultados han indicado que existen múltiples estudios que cuentan con propuestas de modelos integrales para una intervención terapéutica paliativa, pero escasos en terapia de la dignidad. Tras revisar la literatura y comparar los resultados, se ha podido constatar que la aplicación de los Modelos teóricos de la dignidad en la atención al final de la vida es pionera en España. CONCLUSIONES : El concepto de dignidad es tan básico y fundamental que resulta difícil de definir. Según los datos obtenidos, se podría concluir que aún existe un vacío sobre la Dignidad Humana en Cuidados Paliativos. Se ha considerado la necesidad de llevar a cabo una revisión del tema de la Dignidad Humana más cercana a la práctica clínica en Cuidados Paliativos. Es esencial que la Medicina acepte la vulnerabilidad y fragilidad humana

Different reviews that have to world-wide level offer a vision of what is occurring in our country; it has concluded for example that exist an empty between the investigation and the practice of the dignity in Palliative Cares. OBJECTIVES : Review of the literature on the theoretical Models of the human dignity in the attention at the end of the life, and like this give answer to the meaning of dignity, and how preserves, defends and respects in palliative cares. METHODOLOGY : The research was conducted bibliographic research between the months of December and Abril of 2014-2015 in: Pubmed, Google Academic and Scielo. They selected all those articles that adjusted to the question of investigation and fulfilled all the criteria of inclusion.

RESULTS : 155 articles were identified, finally including a total of 32, between them 12 essays. The results have indicated that exist multiple studies that have proposed of integral models for a palliative therapeutic intervention, but scarce in therapy of the dignity. In general, there are few studies in Spain, with theoretical models of dignity in care at the end of life. CONCLUSIONS : The concept of dignity is so basic and fundamental that results difficult to define. According to the data found could conclude that it still exist an empty, on the dignity in palliative cares. It has considered the need to conduct at a review of the subject of the human dignity, nearer to the clinical practice in palliative cares. It is essential that the Medicine accept the vulnerability and human fragility.
