Proyecto de Educación para la salud dirigido a reducir las complicaciones en los pacientes con Insuficiencia Cardíaca de Santa Coloma de Gramenet

Abstract

INTRODUCCIÓN: En la actualidad el aumento de supervivencia de las personas que sufren enfermedades cardiovasculares ha favorecido a la aparición de enfermedades crónicas como la Insuficiencia Cardíaca(IC). En España, la IC es la tercera causa de muerte cardiovascular y en la población más longeva (mayores de 65 años) afecta en gran medida con una prevalencia que oscila sobre el 2% pero aumenta hasta el 10% en mayores de 70 años, también generan una media de 80.000 ingresos de urgencia durante el año. El pronóstico no es más favorable que algunos tipos de cáncer, por ello es importante realizar un buen control de estos pacientes. En Sta. Coloma de Gramenet hay una elevada incidencia y prevalencia de IC y enfermedades cardiovasculares, por ello se ha escogido esta población para ser estudiada. OBJETIVO: En este Proyecto de Educación para la salud se propone como objetivo disminuir un 20% la prevalencia de complicaciones de los pacientes con IC de Sta. Coloma de Gramenet en dos años, a través de la promoción del bienestar y en la normalización de la vida de estos pacientes y de sus cuidadores para que sean capaces de gestionar su tratamiento de forma óptima y segura. MÉTODO: En este Proyecto se proponen dos intervenciones, que tienen como objetivo ofrecer todas las herramientas tanto a pacientes, cuidadores, como a los diferentes profesionales que atenderán a los pacientes y cuidadores. En primer lugar, se realizará un taller de paciente experto dirigido a pacientes y cuidadores. En segundo lugar, se realizará un taller formativo dirigido a la sensibilización y estandarización de cuidados de los profesionales sanitarios de atención primaria. CONCLUSIÓN: La IC es una enfermedad muy prevalente en los países occidentales, generando una elevada mortalidad y un elevado coste a la sanidad debido al gran volumen de descompensación e ingresos que generan. La enfermería es el principal profesional que está al lado del paciente en todo momento, por lo tanto su atención es constante sobre el paciente, familiares y/o cuidadores. Asimismo, estos profesionales mediante las actividades que se plantean pueden favorecer al empoderamiento de la salud y dar conocimientos a pacientes, cuidadores y profesionales.

Coloma de Gramenet in two years, through the promotion of the welfare and the normalization of the life of these patients and their caregivers and they can to be able to manage their treatment safely and optimal. METHOD: In this Project there are proposed two interventions, which take as a target to offer all the tools so much to patients, minders, as to the different professionals who will attend to the patients and minders. First of all, there will be realized a workshop of expert patient directed to patients and minders. Secondly, there will be realized a formative workshop directed to the sensitization and standardization of care of the sanitary professionals of primary health care. CONCLUSIONS: The IC is a very prevalent disease in western countries, generating a high mortality rate and a high cost to the health due to the large volume of decompensation and income they generate. Nursing is the main professional who is the side of the patient at all times, therefore your attention is constant on the patient, family members and/or caregivers. In addition, these professionals through the activities that arise can promote the empowerment of health knowledge and give to patients, caregivers and professionals.
