Estratègies de comunicació amb el nen i la familia al final de la vida: revisió bibliogràfica

Abstract

L'objectiu d'aquest treball és aprofundir en el coneixement de les estratègies infermeres per dur a terme una comunicació terapèutica eficaç amb el nen i la família en situació de final de vida. Els professionals de la salut han d’assumir que el nen i la família són una unitat a tractar en el marc de les cures pal·liatives pediàtriques. La comunicació en el marc de la pediatria pal·liativa requereix seguir unes estratègies específiques ja que els nens, en funció de l'edat, comprenen la malaltia i la mort de forma diferent i passen per diferents etapes maduratives que han de ser conegudes pels professionals. Tanmateix els nens malalts estan molt influenciats pel seu entorn i per les seves experiències prèvies. Les famílies tenen dret a rebre assistència i cura per part de la sanitat, i també d'estar informats degudament de la situació dels seus fills. En aquest àmbit professional, la infermera ha de facilitar que la vida del nen sigui el més normal dintre d'uns límits, fent-lo partícip en la presa de decisions en la mesura del possible. Ha de conèixer els passos a seguir en la comunicació de males noticies, utilitzant unes habilitats específiques i assegurant-se de la comprensió del pacient i la família. Per això és essencial que des de la universitat es realitzi una bona formació en estratègies comunicatives amb el pacient, tant adult com nen, per tal de poder aconseguir uns bons nivells de qualitat en l'atenció sanitària.

The aim of this work is to deepen into the knowledge of the nurses strategies to carry out a therapeutic effective communication with the child and the family in the situation of end of life. The health professionals have to assume that the child and the family are a unit of treatment in the frame of the pediatric palliative care. The communication in the frame of the palliative pediatrics needs to follow a few specific strategies since the children, depending on the age, understand the disease and death in a different way and they go through different maturational stages that have to be known by the professionals. Yet the sick children are very influenced by their environment and their previous experiences. The families have the right to receive assistance and care from the health system, and also of being informed about the situation of his children. In this professional area, the nurse has to facilitate that the life of the child is as normal as possible, within a few limits, making him/her participant in the process of decision making. The nurse has to know the steps to follow in the communication of bad news, using specific skills and ensuring the comprehension of the patient and the family. That's why it is essential a good training from the university in communicative strategies with the patient, both adult and child, in order to obtain good levels of quality in the healthcare system.
