Programa de Educación para la Salud para disminuir los ingresos hospitalarios en pacientes con Insuficiencia Cardíaca en fase III-IV en la Barceloneta

Abstract

INTRODUCCIÓN : El aumento de la prevalencia de la Insuficiencia Cardíaca a nivel mundial es uno de los mayores problemas de salud pública, se asocia a un alto número de ingresos hospitalarios y es responsable de una alta morbimortalidad. Las principales causas de un alto número de ingresos hospitalarios son la baja adherencia terapéutica y la detección tardía de los signos de alarma previos a una descompensación por parte de los pacientes y cuidadores. OBJETIVO : El objetivo de este programa educativo es disminuir ingresos hospitalarios en pacientes con Insuficiencia Cardíaca en fase III-IV en la Barceloneta, para ello se trabajará sobre la adherencia terapéutica y detección precoz de signos de alarma previos a una descompensación. MÉTODO : En este trabajo se propone un Programa de Educación para la Salud, basado en intervenciones educativas, que se realizarán en el Centro de Atención Primaria de la Barceloneta. Dirigidas a pacientes con Insuficiencia Cardíaca en fase III-IV y a sus cuidadores. En primer lugar, se sugieren sesiones prácticas y talleres en las que se abordarán contenidos sobre la adherencia terapéutica y signos de alarma. En segundo lugar, se propone seminarios y talleres dirigidos a familiares y/o cuidador principal para trabajar aspectos relevantes de la patología de la Insuficiencia Cardíaca. CONCLUSIONES : la Educación para la Salud es fundamental para potenciar hábitos saludables educando al paciente en su proceso de recuperación. El personal de enfermería juega un papel importante como promotor de salud y su participación es la clave para un desarrollo eficaz del programa de salud. Consiguiendo el objetivo a corto plazo se puede disminuir ingresos hospitalarios.

INTRODUCTION : The increase of the prevalence of Heart Failure worldwide is one of the major problems of public health; it is associated to a high number of hospital admissions and it is responsible for high morbidity and mortality. The major causes of a high number of hospital admissions are the low therapeutic adherence and the late detection of some warning signs prior to decompensation. OBJECTIVE : The objective of this educational programme is to reduce hospital admissions of patients in phase III-IV of Heart Failure in Barceloneta. To do so it will be worked on therapeutic adherence and early detection of warning signs prior to decompensation. METHOD : In the present work it is suggested a Programme of Education on Health based on interventions, which will take place in the Centro de Atención Primaria (Primary Healthcare Centre), and are addressed to patients in phase III-IV of Heart Failure and their relatives. Firstly, it is suggested practical sessions and workshops in which contents about therapeutic adherence and warning signs will be approached. Secondly, it is suggested seminars and workshops addressed to relatives and /or main carer in order to work on relevant aspects of the Heart Failure pathology. CONCLUSION : Education on health is fundamental to strengthen healthy habits by educating the patient in their recovery process. The nursing staff play a key role as health promoters and their participation is the key for effective development of the health programme. By obtaining the short term objective, the hospital admissions can be reduced.
