La fisioterapia en la ATM y el bruxismo: valoración y tratamiento

Abstract

INTRODUCCIÓN: La articulación temporomandibular (ATM) consta de un complejo sistema músculo-ligamentoso, que forma el sistema masticatorio. Al depender esta articulación de tantas estructuras, es también susceptible de padecer lo que se denomina trastornos temporomandibulares. Una de las afectaciones más comunes de esta articulación es el bruxismo, patología cuya etiología, signos y síntomas no están evidenciados a día de hoy. Es una parafunción oral, de causa multifactorial, que se caracteriza por el apretar (bruxismo concéntrico) y/o rechinar (bruxismo excéntrico) de los dientes de manera involuntaria. OBJETIVOS: Este trabajo pretende poner de manifiesto las influencias más importantes de la ATM para un fisioterapeuta, así como sintetizar todo en lo que al bruxismo respecta en cuanto a causas, valoración y tratamiento. RESULTADOS-PLAN DE ACTUACIÓN: Para el diagnóstico de los pacientes se realiza una valoración musculo-esquelética, precedida de una anamnesis completa, y se utiliza el índice clínico de Maglione para establecer la severidad del caso. Los resultados aportan una clasificación de los pacientes entre afectación leve, moderada o severa. El plan terapéutico, será el mismo para todos los grupos de pacientes. En función de la severidad de la patología, la diferencia se encontrará en la temporalidad en la que se llevará a cabo este. CONCLUSIONES: El bruxismo puede ser causa o consecuencia de trastornos temporomandibulares, y su tratamiento debe ser abordado de una manera global y multidisciplinar.

INTRODUCTION: The temporomandibular joint (TMJ) consists of a complex musculoligamentous system which forms the masticatory system. By relying this articulation of many structures, it is also susceptible to suffer what is called temporomandibular disorders. One of the most common affectations of this joint is bruxism, a pathology whose etiology, signs and symptoms are not evidenced now a day. It is an oral parafunction, with a multifactorial cause, characterized by tightening (concentric bruxism) and / or grinding (bruxism eccentric) involuntarily the teeth. OBJECTIVES: This study aims to highlight the most important influences of the TMJ for a physiotherapist and synthesize particularly as regards bruxism as to causes, assessment and treatment. RESULTS-PLAN OF ACTION: For patients diagnosis, is used a musculoskeletal assessment, preceded by an anamnesis, and clinical Maglione index is used to establish the severity of the case. The results provide a classification of patients with mild, moderate or severe affectation. The treatment plan will be the same for all groups of patients. Depending on the severity of the disease, the difference will be in the timing in which this will take place. CONCLUSIONS: Bruxism can be the cause or the consequence of temporomandibular disorders, and their treatment must be approached in a global and multidisciplinary manner.
