Diagnòstic i tractament de l’esquinç no quirúrgic del lligament lateral extern de turmell i de la neuropatia associada de nivells II-III del nervi peroneal superficial

Abstract

El lligament lateral extern del turmell es lesiona en traumatismes en inversió, i com a conseqüència d’aquests traumatismes poden generar-se neuropaties del nervi peroneal superficial, que innerva de forma motora als músculs peroneals, l’activació dels quals és clau per a la propiocepció del peu i per a evitar les recidives. L’objectiu principal és abordar els esquinços més enllà de l’àmbit estructural, incloent també la valoració i tractament de les possibles lesions neurals derivades dels esquinços de turmell. Els criteris que definiran el diagnòstic seran l’anamnesi (per descobrir el mecanisme lesional i el possible grau d’afecció nerviosa), la palpació (neural i d’estructures músculesquelètiques), tests específics per a l’esquinç, la valoració de la sensibilitat i els tests neurodinàmics. Els resultats de les actuacions diagnòstiques ens porten a classificar 3 grups de pacients: els que pateixen un esquinç de LLE, els que pateixen una neuropatia i els que presenten les dues afectacions. Es desenvoluparà un pla terapèutic per a cada grup. Mètodes com la mobilització precoç, Cyriax i la propiocepció/control motor seran utilitzats per al tractament de l’esquinç. D’altra banda, la neurodinàmica conjuntament amb la propiocepció/control motor s’usaran en l’abordatge de la neuropatia del peroneal superficial. És important no infravalorar els esquinços de LLE, ja que si no es valoren i es tracten correctament, els pacients poden arribar a patir esquinços recidivants i inestabilitat crònica, amb dolor residual. Aquest pla d’actuació garanteix un abordatge complet que evita les situacions prèviament descrites, i pot ser aplicat per qualsevol fisioterapeuta amb coneixement suficient de la matèria.

The ankle lateral external ligament is injured on inversion trauma and, as a result of these injuries, superficial neuropathies can be generated on the peroneal nerve, which innervates in a motor way the peroneal muscles, the activation of which is key to proprioception of the foot and to prevent future recurrences. The main aim of this project is to approach sprains beyond a structural scope, also including the assessment and treatment of neural lesions arising from ankle sprains. The criteria that will define the diagnosis will be the anamnesis (to explore the possible mechanism of injury and the degree of nerve condition), palpation (of neural and musculoskeletal structures), specific tests for sprains, the sensitivity assessment and the neurodynamic tests. The results of the diagnostic performances lead us to classify patients in three groups: those suffering a sprained lateral ligament, those who suffer from neuropathy and those with both affectations. A therapeutic plan for each group will be developed, using methods such as early mobilization, Cyriax and proprioception/motor control for the treatment of sprains. Furthermore, neurodynamic, along with the proprioception/motor control will be used in the approach of the neuropathy of the superficial peroneal. It is important not to underestimate the lateral external ligament sprains, because if not valued and treated properly, patients may suffer recurrent sprains and chronic instability, with residual pain. This action plan guarantees a complete approach that will avoid the situations previously described, and can be applied by any therapist with enough knowledge about the matter.
