Neuropatia del Nervi de Baxter: Valoració i Tractament de Fisioteràpia

Abstract

INTRODUCCIÓ: El dolor de taló és una condició clínica que pot ser deguda a una amplia gamma etiològica. Tot i no ser tant comú, aquest dolor pot ser degut a neuropaties focals secundàries a un traumatisme o a un atrapament. Concretament, la revisió es centra en la neuropatia del nervi de Baxter, una síndrome que resulta de l’afectació de la primera branca del nervi plantar lateral. OBJECTIUS: Aquesta patologia encara roman desconeguda per a un gran nombre de professionals, motiu pel qual s’ha decidit aprofundir en el seu diagnòstic i posterior tractament. RESULTATS – PLA D'ACTUACIÓ: Es portarà a terme un diagnòstic basat en proves clíniques, tals com la palpació, tests neurodinàmics, etc., juntament amb les proves complementàries que s’escaiguin. Com a resultat de l’actuació diagnòstica, s’espera trobar pacients amb neuropatia del nervi de Baxter distribuïts en diferents graus de severitat (agut, subagut i crònic). En el present pla d’actuació terapèutica es proporcionarà un tractament als pacients diagnosticats de neuropatia del nervi de Baxter, constituint grup de tractament aquells que es trobin en un estadi subagut i crònic. De cara al desenvolupament del pla es tindran en compte els factors etiològics, agreujants i perpetuadors, així com els trets característics de cada pacient. CONCLUSIONS: el pla diagnòstic plantejat permetrà tenir en compte la neuropatia del nervi de Baxter com a possible causa del dolor de taló, una entitat freqüentment mal diagnosticada. Tanmateix, el pla terapèutic constituirà una eina específica i detallada que permetrà als professionals de la Fisioteràpia abordar correctament aquesta neuropatia.

INTRODUCTION: Plantar heel pain is a clinical condition that can be originated by a wide range of etiologies. Although it is a less common condition than the others, this pain may also be caused by focal neuropathies due to entrapments or traumatisms. Specifically, this review focuses on Baxter’s nerve neuropathy, which is a syndrome resulting from the involvement of the first branch of the lateral plantar nerve. OBJECTIVES: This disease still remains unknown to a large number of professionals, which is the main reason to go in depth into its diagnosis and posterior treatment. RESULTS–PROCEDURE PLAN: A diagnosis based on clinical trials will be carried out, such as palpation, neurodyamics tests, etc. In addition, appropriate complementary tests should be carried out when it is necessary. As a result of the diagnostic performance it is expected to split patients suffering from Baxter’s nerve neuropathy, into different grades of severity (acute, subacute and chronic). In this therapeutic plan, a useful treatment will be provided to each patient with a Baxter’s nerve neuropathy diagnostic. Specifically the patients in a subacute and chronic stage become the treatment group. To the development of the plan, etiologic, aggravating and perpetuating factors, as well as the patient individual features, will be considered. CONCLUSIONS: The diagnostic plan will allow considering Baxter’s nerve neuropathy as a possible cause of plantar heel pain, as it is an entity frequently misdiagnosed. Furthermore, the therapeutic plan will constitute a specific and detailed tool, allowing the Physiotherapist to correctly address this neuropathy.
