Relació del dolor a la cara externa del turmell per afectació del nervi peroneo superficial i endometriosis. Valoració i tractament

Abstract

Les entorsis de turmell, a través del seu mecanisme lesional d’inversió i flexió plantar de turmell, poden generar afectacions a nivell distal de l’extremitat inferior, sent la compressió del nervi peroneo superficial una de les conseqüències més freqüents. Poques vegades és pensa que una alteració musculesquelètica pot veure’s influenciada a nivell somatovisceral per un òrgan, el qual, comparteixi la mateixa innervació simpàtica. El turmell comparteix la mateixa innervació simpàtica amb l’ovari, entre d’altres. Una afectació de l’ovari com és l’endometriosis, que és caracteritza per la presencia de teixit endometrial fora de la cavitat uterina, pot influir en el dolor de la cara externa del turmell. En aquest cas per afectació del nervi peroneo superficial després d’una entorsis de turmell. La via del segment facilitat, per mitjà d’impulsos irritatius que arriben a la medul·la i pel control, per via simpàtica, que exerceixen els segments vertebrals sobre uns altres segments, ens mostra que existeix una relació que vincula el sistema visceral amb afectacions musculesquelètiques. Respecte a la valoració, haurem de realitzar una exploració visual i palpatòria de les diferents estructures que creiem que poden estar lesionades. Valorarem a nivell muscular, neurològic i neurodinàmic, per comprovar l’estat en el que es troba el nervi peroneo superficial. A més a més, farem una exploració del sistema nerviós autònom i de la pelvis menor. Les proves complementàries d’imatge, completaran i confirmaran el nostre diagnòstic. El tractament de fisioteràpia haurà d’incloure l’aspecte neurodinàmic pel nervi peroneo superficial, l’entorsis en la seva fase aguda, l’endometriosi a nivell visceral i en conseqüència el sistema nerviós autònom.

Ankle sprains, through their injury mechanism of plantar flexion and ankle inversion, can affect the proximal lower limb and cause the compression of the superficial peroneal nerve, being this one of the most common consequences. It is very uncommon to think that a musculoskeletal disorder may be influenced at the somato-visceral level by an organ which shares the same sympathetic innervation. The ankle and the ovary share the same sympathetic innervation. An alteration in the ovary such as the endometriosis, which is characterized by the presence of endometrial tissue outside the uterine cavity, affects the alteration of the superficial peroneal nerve after suffering a first degree sprain. The facilitated egment –using irritative impulses that reach the spinal cord and because of the control (via sympathetic channels) carried out by the vertebral segments on other segments– shows a link between the visceral system with the musculoskeletal alterations. Regarding the assessment, we proceed with a visual and palpatory examination of the different structures that we believe may be injured. We will look into the muscular, neurological and neurodynamic aspects in order to check the state of the superficial peroneal nerve. In addition , we will examine the autonomic nervous system and minor pelvis. The complementary image tests will complete and confirm our diagnosis. The physiotherapic treatment should include the neurodynamics of the superficial peroneal nerve, the ankle sprain in its acute phase, the endometriosis at visceral level, and therefore/consequently the autonomic nervous system .
