Disfuncions de l’Articulació Temporomandibular: Diagnòstic diferencial i tractament en fisioteràpia

Abstract

Els trastorns de l’articulació temporomandibular (TTM) són considerats segons la literatura científica un problema significatiu en l'àmbit de la salut pública. Les disfuncions estan caracteritzades clínicament per dolor orofacial, limitació funcional, bloquejos i sorolls articulars. El diagnòstic i l’abordatge terapèutic d’aquesta alteració és complexa degut a que l’etiologia és multifactorial. Els objectius principals són conèixer i estudiar les característiques principals de l’articulació, fent incidència en l’epidemiologia, la fisiopatologia, l’abordatge des dels principals punts de vista clínics i l’aprofundiment en l’abordatge fisioterapèutic. A través de la recerca científica realitzada, es planteja com elaborar un pla de diagnòstic eficaç i global. Les dades proveïdes a partir dels signes i símptomes, juntament amb la informació narrada pel pacient durant l’exploració clínica serviran per enfocar el tractament d’una manera personalitzada. S’exposen tres plans d’actuació terapèutica. Un primer on es proposa un tractament aplicable a pacients que presentin molèsties de la regió de l’ATM, però que no resulta específic per a cap patologia concreta. Aquest comprèn des de temes merament d’educació fins a teràpies manuals. El fet de no ser específic és un factor que permet combinar-lo fàcilment amb els altres dos. El segon exposa un tractament específic per l’abordatge del dolor miofascial, basat principalment en la desactivació dels punt gallet miofascials. Per últim, un tercer pla que estarà indicat per pacients amb patologies articulars i discals. El terapeuta serà l’indicat d’escollir en cada cas quin tractament realitzar i de quina manera combinar-lo.

Temporomandibular joint disorders (TMD) has been considerated a significant problem in public health. Dysfunctions are characterized by orofacial pain, functional limitation, articular noises and blocks. Moreover, diagnosis and approach are difficult due to multifactorial etiology. The aims of this research are to understand and study the principal features of the TMJ, its epidemiological impact in the general population, the physiopathological aspects of the disease, the current approach from the main clinical points of view and to analyze the physical therapy approach in depth. An effective and integrative diagnosis plan is considered to be developed through the purposed scientific research. Provided data obtained from signs and symptoms together with patient’s explanation during the anamnesis will be used to create a personalized treatment. There are three therapeutic action plans exposed in this dissertation. The first one is a general plan that can be applied to any patient who has some discomfort in the TMJ region, but it isn’t purposed to treat a specific temporomandibular pathology exclusively. This plan includes from manual therapy techniques to self-care advices. To be a non-specific treatment allows it to be used in combination with the other two therapeutic plans easily. The second approach is based on myofascial trigger point release techniques to deal with myofascial pain syndrome in the TMJ. Finally, the third and last action plan would be indicated for articular and disc disorders like TMJ derangements, for example. The therapist will have to choose the right therapeutic plan and techniques depending on the pathology.
