La síndrome del canal carpià en relació al dolor cervical crònic, concretament radiculopaties cervicals

Abstract

La síndrome del canal carpià és una neuropatia per atrapament del nervi mitjà en el canal carpià, més freqüent en dones en la cinquena dècada de vida. Es creu que els factors predisposants més importants són de caràcter mecànic. En el cas del dolor cervical crònic, es tracta d'una patologia multicausal, sent una de les causes les radiculopaties, on s'ha trobat una possible relació associativa amb la SCC. La radiculopatia cervical, és una lesió de l'arrel nerviosa on es pot veure afectat el dermatoma, l'esclerotoma i el miotoma. Durant la pràctica clínica s’ha observat que molts dels pacients que pateixen una SCC, presenten símptomes a la zona cervical, i viceversa. Aquesta relació es pot deure a la síndrome de double crush, que explica la possible aparició d'ambdues patologies a causa del deteriorament del transport axonal. L'objectiu principal d'aquest protocol és eliminar la simptomatologia del pacient, elaborant un abordatge terapèutic exhaustiu per als pacients que presentin les dues patologies. La intenció és aconseguir una recuperació favorable i ràpida, sense perdre de vista la individualització de cada patologia, i d’aquesta manera, millorar la qualitat de vida del pacient. Aquesta guia consta d'una explicació detallada, amb la classificació corresponent segons el nivell de simptomatologia, així com els tests específics per valorar el grau de la patologia. A més d'una bona anamnesi per no donar lloc a un diagnòstic erroni. Cada tractament ve dictaminat pel grau i evolució en el que es trobi el pacient, adaptant les tècniques a aplicar.

The Carpal Tunnel syndrome is a neuropathy for entrapment of the median nerve in the carpal tunnel, more frequent in women in their fifth decade of life. It is thought that the most important predisposing factors are of mechanical character. In the case of chronic neck pain, this is a multicausal pathology – radiculopathies being one of the causes, where a possible associative relation with the carpal tunnel syndrome has been found. Cervical radiculopathy is an injury of the nerve root that can affect the dermatome, myotome and sclerotome. During the clinical practice it was observed that some of the patients who have a carpal tunnel syndrome also have symptoms in the cervical zone, and vice versa. This relation might be due to the double crush syndrome, which explains the possible appearance of the two pathologies because of the deterioration of the axonal transport. The main objective of this protocol is to erase the symptomatology of the patient, making an exhaustive therapeutic approach for the patients who present the two pathologies. The intention is to obtain a favourable and quick recovery, without ignoring each pathology itself, to improve the patient’s quality of life. This guide contains a detailed explanation, with the correspondent classification depending on the level of the pathology, as well as the specific tests to evaluate the grade of the pathology. Additionally, a good anamnesis not to give a wrong diagnostic is included. Each treatment is ruled by the grade and evolution of the patient, adjusting the techniques to apply.
