Evaluación de la agudeza táctil lumbar en pacientes con lumbalgia aguda y su efecto sobre el dolor, imagen corporal y movilidad lumbar

Abstract

Introducción: Numerosos cambios en la función sensorial y motora se han descrito previamente en casos de dolor lumbar crónico. Entre ellos se han descrito alteraciones en la Agudeza Táctil y la Imagen Corporal. Objetivos: A través del Test de Discriminación de Dos Puntos, encontrar cambios centrales relacionados con la Agudeza Táctil en pacientes con dolor lumbar en fase aguda/subaguda. Adicionalmente determinar la relación de dicho test con variables como dolor, imagen corporal o función biomecánica. Metodología: Fueron reclutados 24 participantes, de los cuales 12 formaron el grupo experimental y otros 12 el grupo control. Se tomaron medidas en una única sesión, las cuales fueron la EVA, el Test Dedos-suelo, The Freemantle Back Awareness Questionnaire y el Test de discriminación de dos puntos, además de datos característicos de cada sujeto. Resultados: Se encontraron diferencias significativas relativas a la Agudeza Táctil lumbar entre los grupos, indicando que el grupo experimental muestra peor capacidad discriminativa. También se encontraron diferencias significativas en el Test Dedos-suelo entre los momentos pre y post TPDT, reflejando mejoría. Por el contrario no se encontraron diferencias significativas relativas a la autopercepción corporal. Discusión: Alteraciones en la capacidad para discriminar dos puntos son objetivables en aquellos pacientes que se encuentran en fase aguda/subaguda de la condición de dolor lumbar.

Introduction: There are many descriptions about changes in the motor and sensory function regarding to chronic low back pain. These alterations are described concerning the tactil acuity and body image. Objectives: Firstly to objectify central changes related to Tactil Acuity, using the test in patients in the acute/subacute LBP phase. Secondly to determine the efficacy of the test concerning pain, selfperception and biomechanical function. Methods: There were assessed a total of 24 subjects. 12 were part of the experimental group, and 12 of the control one. During a single session, different measures were taken, as the VAS, the Fingers- floor Test, The Freemantle Back Awareness Test Questionnaire and two-point discrimination, in addition to characteristic data of each subject. Results: Significant differences were found concerning the lumbar variables, indicating that the experimental subjects have poor tactile acuity compared to the control group. Also, significant differences related to the distance between fingers and the floor in the pre and post moments were found. No significant differences were found in the case of the self-perception. Discussion: Changes or alterations related to the ability to discriminate two points are objetctified in those patients who are in the acute/subacute phase of LBP condition.
