Patrons evasius en l’hemiplegia: Patoneurodinàmica i abordatge terapèutic

Abstract

Mitjançant una revisió sistemàtica de la bibliografia present en llibres, revistes i bases de dades científiques, es proposa l’objectiu de conèixer l’origen dels patrons espàstics i la patoneurodinàmica que hi pugui estar implicada. Es revisa quines són les alteracions de l’ictus que poden afectar als mecanismes fisiològics de d’adaptació neural, a més de plantejar una valoració i un abordatge terapèutic d’acord amb els factors patològics implicats. Els resultats de la revisió indiquen que falta molta evidència científica per detectar aquests patrons patoneurodinàmics en el pacient amb ictus. Malgrat això, la pràctica clínica habitual avala la seva existència, i molts autors ja comencen a parlar de la presència d’aquests patrons en els pacients amb lesió neurològica. Finalment, es realitza un protocol d’actuació en el que es proposa l’aplicació de tècniques de mobilització neural amb tècniques específiques pel sistema nerviós autònom.

The objective to know the origin of spastic patterns and pathoneurodynamics that may be involved is set through a systematic revision of the bibliography found in books, journals and scientific databases. It is meant to revise what changes of the stroke can affect physiological mechanisms of the neural adaptation, as well as raise an assessment and treatment approach according to pathologic factors involved. The results of the revision indicate that there is a severe lack of scientific evidence to detect these pathoneurodynamic patterns in stroke patients. However, clinical practice supports its existence, and many authors have started to talk about the presence of these patterns in patients with neurological injury. Finally, there is a protocol in which it is proposed the application of neural mobilization techniques with specific techniques for autonomic nervous system.
