Hipotèrmia perioperatòria inadvertida: revisió bibliogràfica

Abstract

Des de fa anys, diferents organismes internacionals que vetllen per la qualitat de les cures, han creat protocols per millorar la seguretat del pacient al bloc quirúrgic, entre els quals hi ha els que fan referència al manteniment de la normotèrmia per tal de millorar els resultats dels pacients. Es coneix per hipotèrmia, la temperatura corporal central menor a 36ºC, essent un problema de salut actual i freqüent que pateix entre el 72-90% dels pacients quirúrgics segons diversos estudis realitzats. Aquesta complicació, ha d’ésser evitada per tal de reduir els efectes adversos que comporta, els quals deriven en un alt impacte econòmic. La temperatura corporal, és un paràmetre fisiològic que es pot mesurar igual que la pressió arterial, saturació d’oxigen, freqüència cardíaca i respiratòria. Aquests paràmetres, a diferència de la temperatura, tenen un alt índex de seguiment i monitoratge i permeten detectar de manera immediata canvis que puguin afectar la seguretat del pacient. La temperatura corporal per altra part, no compta amb un seguiment tan estricte i com a resultat, es produeixen estats d’hipotèrmia, que poden passar desapercebuts perquè cursen de manera subclínica a l’organisme. Aquest treball, mostra una revisió bibliogràfica del concepte d’hipotèrmia perioperatòria involuntària, així com descriure la fisiologia i mecanismes de la termoregulació, efectes adversos i diferents estratègies per a la prevenció i tractament de la hipotèrmia durant tota l’etapa perioperatòria. Since many years ago, different international organisms that look after for quality care have created protocols to improve patient safety in the operation room area, some of them concerned to the maintenance of the normothermia to improve patient outcomes. It is known as hypothermia, a drop in the body temperature under 36ºC. This is a current and frequent health problem that affects between 72-90% of the surgical patients according to several studies. This complication must be avoided in order to reduce the adverse effects involved that will result in a high economic impact. Body temperature is a physiological parameter, just as blood pressure, oxygen saturation, heart rate and breathing. All these settings, unlike body temperature, have a high level of tracking and monitoring and immediately allow detecting changes that may affect patient safety. Body temperature measurement does not have a strict follow-up and as a result of this, hypothermia appears, sometime in a subclinical way, with no signs of hypothermia observed. This work shows a bibliographic review about the concept of involuntary perioperative hypothermia and describes the physiology and thermoregulation mechanisms, the adverse effects as well as the different strategies for the prevention and treatment of hypothermia during the perioperative period.
