Uso de la terapia de presión negativa VAC® en la reducción del riesgo de infección y de dehiscencia en las heridas quirúrgicas: revisión de la bibliografía

Abstract

PROPÓSITO Se han demostrado efectos positivos al usar la terapia de presión negativa (TPN). Las revisiones sistemáticas se han puesto de moda, pero aún hay mucho desconocimiento sobre sus efectos en heridas quirúrgicas curando por primera intención. El objetivo és averiguar si la TPN, reduce el número de complicaciones relacionadas con las incisiones quirúrgicas, concretamente el riesgo de infección y dehiscencia, en heridas limpias y cerradas. MÉTODOS Este estudio siguió las indicaciones de la guía PRISMA y consiste en una revisión bibliográfica de revisiones sistemáticas. Las revisiones sistemáticas escogidas fueron las publicadas en los últimos cinco años y que usaban la TPN para curar heridas quirúrgicas en primera intención. La búsqueda se realizó en la base de datos de Medline, usando del motor de búsqueda de Pubmed, durante enero de 2016. RESULTADOS La TPN no debe ser usada en pacientes ortopédicos debido a la alta incidencia de ampollas en la piel. Aparte de en este grupo de pacientes, resulta una terapia segura, bien tolerada y que ofrece beneficios para los pacientes, pero debido a su elevado coste, existe una necesidad de innovar en estos dispositivos para reducir su coste y poder generalizar su uso. CONCLUSIONES Las complicaciones de dehiscencia de la herida e infección suponen unos gastos extraordinariamente caros, debido a que requieren antibiótico endovenoso e incluso en ocasiones reintervenciones. Debido a este elevado coste, existe la necesidad de analizar estudios de alta calidad para determinar en qué ocasiones se debe utilizar. PURPOSE Positive effects have been reported after use of negative pressure wound therapy (NPWT). Systematic reviews have become popular, but little is known about their efficacy in surgical wounds healing by primary intention. The aim was to investigate if NPWT, reduce the complications related with surgical wounds, concretely the risk of infection and dehiscence over clean and closed incisions. METHODS This study follows the PRIMSA guidelines directions and is a bibliographic review of two systematic reviews. The systematic reviews chosen were the published in the last five years and use the NPWT to heal the surgical wounds in primary intention. The search was done in the data base Medline using the motor search Pubmed in January of 2016. RESULTS The NPWT should not be used in orthopedic patients due the high incidence of skin blistering. A part of this group of patients is safe, well tolerated and offers benefits for patients, but because of its high cost, there is a need for innovation in these devices to reduce their costs and be able to generalize the use. CONCLUSIONS Wound dehiscence and infection are extraordinarily expensive complications that require intravenous antibiotic and frequently additional surgery. Because of its high cost, needed to analyse high quality trials to size up the effects of the newer products of NPWT to determine in which occasions should be used.
