Treball de l’equilibri amb doble tasca en ictus: indicacions i pautes de intervenció

Abstract

INTRODUCCIÓ: L‟ictus és una de les principals afectacions en termes de mortalitat, incidència i discapacitat a nivell mundial, per tant el treball de rehabilitació serà un punt important a tenir en compte després d‟haver-lo patit. Un dels factor més discapacitant és la falta d‟equilibri i a través del seu treball a partir de la doble tasca és un factor essencial per automatitzar el control postural en situacions de la vida diària. OBJECTIUS: Establir un protocol de valoració i treball de l‟equilibri utilitzant la doble tasca en pacients amb ictus per millorar la estabilitat tant en sedestació com en bipedestació i marxa. RESULTATS – PLA D‟ACTUACIÓ: A través de les revisions, s‟han seleccionat com a “pacient tipus” aquells amb un mínim de control de tronc, amb una puntuació de 2 o més en el Trunk Impairment Scale (TIS) en sedestació, amb un nivell cognitiu mínim de 20 o més en la Montreal Cognitive Assessment (MoCA), i una puntuació normal (0-4) pel que fa a la depressió amb el test de Yesavage . Se‟ls aplicarà un protocol de treball en doble tasca per mantenir aquest equilibri de forma automàtica millorant/aconseguint així la seva estabilitat en les diverses posicions de treball. CONCLUSIONS: Creiem que després de la revisió realitzada, el treball de la doble tasca per automatitzar l‟equilibri serà el més indicat per recuperar el control postural perdut reduïnt la necessitat de mantenir l‟atenció en l‟equilibri. INTRODUCTION: A stroke is one of the most serious affections when talking about mortality, incidence and disability worldwide, so the rehabilitation work will be an important point to bear in mind after a stroke has been suffered. One of the major disability factors is the lack of balance and working on this with dual task is an essential factor to automate postural control in daily situations OBJECTIVES: To establish a protocol for assessing and balancing work using dual task in stroke to improve stability in both seated position, standing and gait. RESULTS - PLAN: Through reviews, we have chosen as "patient type" those who have a minimum trunk control in the seated position, scoring 2 or more on TIS, at least 20 or more on MoCA for cognitive level and in relation to depression we need a score of 4 or less in the Yesavage test. We apply a working protocol dual task to maintain this balance automatically improving stability in every position proposed. CONCLUSIONS: After the review, we believe that, the work of the dual task to automate the balance will be the most suitable for recovering lost postural control without the need to maintain balance taking attention.
