Mareo Cervicogénico: Evaluación y Tratamiento desde la Fisioterapia

Abstract

INTRODUCCIÓN: El mareo cervicogénico (MC) es una patología con origen en la región cervical. Su principal síntoma es el mareo, el cual puede ser debido a un gran cantidad de disfunciones. Este hecho, en conjunto con una etiopatogenia no muy específica, hacen que el diagnóstico diferencial sea el mayor reto en esta patología. OBJETIVOS: El marco científico de esta patología no presenta una estructura concreta; de este modo el principal objetivo de este plan de actuación es profundizar y organizar a valoración y el tratamiento del tema en cuestión. RESULTADOS – PLAN DE ACTUACIÓN: Se lleva a cabo una valoración basada en la clínica, los test específicos para el MC y las pruebas complementarias, siempre y cuando estas sean necesarias. Posteriormente al diagnóstico diferencial, podemos dividir el MC en tres subgrupos: Mareo propioceptivo, Vértigo Rotacional de la Arteria Vertebral y Síndrome de Barré-Liéou. En el plan de actuación terapéutica se presenta un tratamiento específico para los pacientes que presentan MC de origen propioceptivo. En ocasiones concretas, los pacientes de los otros dos subgrupos, también son candidatos válidos para la realización de este plan de actuación terapéutica. De este modo se ofrecen un tratamiento y un seguimiento que se adaptan al origen del mareo y a las características del paciente. CONCLUSIONES: El plan de actuación diagnóstica permite a los profesionales de la salud identificar de manera ordenada cual es el origen de la sintomatología, y si es posible el tratamiento de fisioterapia. El plan de actuación terapéutica se define como herramienta útil y específica en el abordaje del MC. INTRODUCTION: Cervicogenic dizziness (CD) is a pathology that has its origin in the cervical region. Its principal symptom is the dizziness, which can be due to different dysfunctions of the body. This fact, in addition to a non-specific etiopathogenesis, makes the differential diagnosis as one of the most difficult parts of this pathology. OBJECTIVES: There is not a specifically determined scientific framework of this pathology, thereby, the main objective of this procedure plan is to deepen and organise the evaluation and treatment of the subject. RESULTS - PROCEDURE PLAN: It will be carried out a clinic-based evaluation, specific test for the CD and diagnostic imaging, when necessary. Posterior to the differential diagnosis, the CD can be classified in three different subgroups: Proprioceptive Cervical Vertigo, Rotational Vertebral Artery Vertigo and Barré-Liéou Syndrome. The therapeutic plan presents a specific treatment for the patients with Proprioceptive Cervical Vertigo. In some concrete situations, patients suffering from the other two subgroups of CD will be valid candidates for this therapeutic plan too. In this way, a dizziness-origin and patient characteristics adjusted treatment and monitoring is offered. CONCLUSIONS: The diagnostic plan allows all the health professionals to identify the real causes of the symptomatology in an organized way. It also allows them to decide if it's possible to perform the physiotherapist treatment. In addition, the therapeutic plan is a useful and specific tool in the approach of the CD.
