Efectividad de la fisioterapia en los síntomas a nivel de la mano en pacientes con artritis reumatoide

Abstract

La artritis reumatoide (AR) es una enfermedad inflamatoria crónica caracterizada por dolor e hinchazón de las articulaciones, disminución de la sensibilidad y destrucción de la sinovial articular, llevando al paciente a una severa incapacidad. El proceso autoinmune en la AR depende de la activación de las células inmunes, que usan quinasas intracelulares para responder a estímulos externos que contribuyen a la trasformación agresiva del fenotipo de los linfocitos B y al desarrollo de un intenso infiltrado cuyo resultado final es la destrucción del cartílago y el hueso. El objetivo principal de este trabajo, tras la realización de una búsqueda bibliográfica exhaustiva es el del planteamiento de un tratamiento fisioterapéutico para los pacientes que padecen AR en fase crónica. Se tratará a pacientes a los que tras realizar el diagnostico según los criterios de la 2010 ACR/EULAR y una exploración articular, se les valorará el grado de dolor (EVA), deformidad (goniometría y Rx) y disminución de la funcionalidad y de la fuerza de agarre (pinzas, balance muscular). El tratamiento dependerá de cuál de los anteriores síntomas predomine. Constará de un plan a realizar en la consulta (terapia manual, masaje y ejercicios) y una pauta a implementar por el paciente en su domicilio (automasaje y ejercicios) acompañada de ortesis. Con estos tratamientos esperamos que disminuya el dolor, la deformidad y que mejore la funcionalidad y la fuerza de agarre, consiguiendo que así el paciente pueda tener una mejor calidad de vida. Rheumatoid arthritis (RA) is a chronic inflammatory disease characterized by pain and swelling of the joints, decreased sensitivity and destruction of the joint synovial, leading the patient to severe disability. The autoimmune process in RA depends on the activation of the immune cells, which use intracellular kinases to respond to external stimuli that contribute to the aggressive transformation of the B lymphocites’ phenotype and to the development of an intense infiltrate whose final result is the destruction of the Cartilage and bone. The main objective of this work, following an exhaustive bibliographical search, is the approach of a physiotherapeutic treatment for patients with RA in the chronic phase. Patients who, after performing the diagnosis according to the 2010 ACR / EULAR criteria and a joint exploration, will be assessed for the degree of pain (EVA), deformity (goniometry and Rx) and decreased functionality and Grip strength (tweezers, muscle balance). The treatment will depend on which of the above symptoms predominates It will consist of a plan to perform in the consultation (manual therapy, massage and exercises) and a pattern to be implemented by the patient at home (self-massage and exercises) accompanied by orthotics. With these treatments, we hope to reduce the pain, the deformity and improve the functionality and the grip strength, achieving that the patient can have a better quality of life.
