Valoració i abordatge en fisioteràpia de l’entorsi lateral de turmell de grau I amb o sense afectació del nervi peroneal superficial de nivell 2 i 3

Abstract

INTRODUCCIÓ. L’esquinç de turmell és una de les afectacions més comunes en l’àmbit esportiu i recreatiu, i el complex lligamentós lateral és el que més freqüentment es lesiona. L’entorsi lateral de poca gravetat és sovint subtractada i menystinguda en el seu estadi agut. L’associació de l’entorsi amb l’afectació del nervi peroneal superficial es creu que és rara, però pot no ser tan estranya com la literatura suggereix. La revisió se centra en l’entorsi aguda i la seva possible implicació nerviosa. OBJECTIUS. Detectar i analitzar la implicació del nervi peroneal superficial i la seva correlació amb l’entorsi lateral. Elaborar un pla terapeutic consensuat d’ambdues afectacions. RESULTATS I PLA D'ACTUACIÓ. Es durà a terme un diagnòstic basat en l’anamnesi, els signes i símptomes clínics observats i en tests específics. Com a resultat, s’espera trobar pacients amb entorsis de grau I agudes o avançades, tant amb afectació neural com sense. Posteriorment, a aquest grup se’ls aplicarà un tractament fisioterapèutic per a conduir-los finalment a l’alta, derivant tots aquells que ho requereixin. CONCLUSIONS. Aquest pla diagnòstic permet la inclusió tant del nervi peroneal superficial com del diagnòstic diferencial acurat de l’entorsi. El pla d’actuació optimitza el procès de curació i prevé la cronificació de l’esquinç. Aquest pla pot servir de guia o suport per a tots els professionals de la salut per tractar correctament l’entorsi de turmell aguda o avançada. NTRODUCTION. Ankle sprain is one of the most common affections in sports and recreation, with the lateral ligament complex being the most frequently injured. Lateral sprain of low severity is often under-treated and under-appreciated in its acute stage. The association of the sprain with the involvement of the superficial peroneal nerve is believed to be rare, although it is usually not as unusual as the literature suggests. The review focuses on acute sprain and its possible nerve involvement. OBJECTIVES. To detect and analyze the involvement of the superficial peroneal nerve and its correlation with the lateral ankle sprain. To elaborate a consensuate therapeutic plan of both affectations. RESULTS - ACTION PLAN. A diagnosis based on anamnesis, clinical signs and symptoms and specific tests will be carried out. As a result, it is expected to find patients with acute or advanced grade I ankle sprains, both with and without neural involvement. Subsequently, physiotherapeutic treatment will be applied to this group to finally lead to discharge, deriving to all of those who require it. CONCLUSIONS. This diagnostic plan allows the inclusion of the superficial peroneal nerves and the precise differential diagnosis of the sprain. The action plan optimizes the healing process and predicts the sprain chronicity. This plan can serve as a guide or support for all health professionals to properly treat acute or advanced ankle sprain.
