Valoració abordatge en Fisioteràpia del dolor lumbar crònic de causa no específica

Abstract

INTRODUCCIÓ: El dolor lumbar crònic inespecífic mostra una gran prevalença a nivell mundial, d’entre un 59-84%. És interessant l’estudi d’un pla d’actuació terapèutic ja que el NSLBP suposa un elevat cost sanitari. La recuperació d’aquests pacients s’ha vist dificultada per un diagnòstic pobre amb uns tractaments estandarditzats i inespecífics. Això, és degut a que no existeix un protocol d’actuació clínic publicat, validat i consensuat. OBJECTIUS : El principal objectiu diagnòstic és dur a terme un diagnòstic diferencial per a pacients amb NSLBP, per a posteriorment realitzar el pla terapèutic més específic possible d’acord amb l’etiologia del seu dolor i disfunció. Per altra banda, l’objectiu terapèutic és restablir el control motor com a punt clau comú en el tractament. RESULTATS – PLA D'ACTUACIÓ : El pla d’actuació diagnòstic s’elabora a partir dels signes i símptomes que relatarà el pacient a l’anamnesi. Seguidament, es realitza l’exploració per així poder diferenciar la possible causa del dolor. Generalment, obtindran un dèficit de control motor. No obstant, també s’haurà de concloure si hi ha una alteració de la mobilitat. Així doncs, segons el predomini d’aquests dos dèficits es realitza la classificació. A continuació, es duu a terme el pla d’actuació terapèutic. En aquests, hi ha una part comuna de treball de control motor, i una altra diversa pel que fa a la teràpia manual i els exercicis específics. CONCLUSIONS : Per concloure, serà de vital importància tan un bon pla diagnòstic com terapèutic primerament per a intentar solucionar de manera progressiva el problema inicial i, finalment per evitar futures recidives. INTRODUCTION: Chronic nonspecific lower back pain disorder shows a high prevalence across the world, ranging from 59 to 84%. The study of a therapeutic plan of action is highly relevant since the NSLBP supposes elevated health care costs. The recovery of these patients has been compromised by poor diagnostics and a nonspecific, standardized treatment. This is due to the fact that there is no protocol of clinic action that has been established, validated and agreed on. OBJECTIVE: The main diagnostic objective is to conduct a differential diagnostic for patients with NSLBP, so that then the most adequate therapeutic plan can be chosen based on the etiology of their pain. On the other hand, the therapeutic objective is to reestablish the motor control, which is the essential part of the treatment. RESULTS - PLAN OF ACTION : The plan of action will be carried out on the basis of diagnostic signs and medical conditions that will be explained by the patient in the anamnesis. Next, the study of the patient will determine the possible causes of the pain. Usually all patients suffer from a deficit of motor control, however, it also has to be determined if the patient suffers from a deficit in mobility as well. Based on these two deficits the evaluation will be performed. The next step is to carry out the therapeutic plan of action. In this plan there is a common part related to the motor control, and a diverse part with regard to manual therapy and specific exercises. CONCLUSION: In conclusion, it will be of vital importance to have a good diagnosis and treatment plan to gradually solve the initial problem and, finally, to avoid future recurrences.
