Análisis del impacto emocional del uso de la contención mecánica en profesionales de Enfermería: revisión bibliográfica

Abstract

Actualmente, existen determinados momentos en los trastornos mentales graves y no tan graves, así como en situaciones de riesgo de caídas, lesiones, etc. que generan una obligación ético-legal, sobre el profesional sanitario, de hacer uso de todos aquellos medios terapéuticos que se encuentran disponibles, incluso llevar a cabo intervenciones que restringen la autonomía y la libertad del paciente mental, como es la contención mecánica. El uso de esta medida por parte del personal de enfermería, provoca un impacto no únicamente a nivel físico, sino también emocional. Por ello, el objetivo de investigación de este estudio es conocer cómo afecta el uso de la contención mecánica a nivel emocional sobre el profesional de Enfermería. Por otro lado, se analiza el conocimiento que posee el personal acerca del uso de la medida, ya que puede suponer un factor causante o agravante de las sensaciones negativas que genera el manejo de la restricción física. La metodología utilizada fue de revisión bibliográfica. El uso de la contención mecánica genera un impacto negativo a nivel emocional, sobre los enfermeros/as de las unidades de salud mental y de geriatría. Los profesionales sienten miedo, pena, compasión, culpa, angustia y miedo de abusar de los derechos de los pacientes. Llevar a cabo una contención puede ser angustioso y puede causar ansiedad en el personal, así como también, ira tristeza, preocupación, frustración, shock, etc. La contención mecánica es una medida en la cual los enfermeros/as se sienten incómodos y, además, la perciben como una necesidad para la que no hay alternativa. Por otro lado, se han encontrado que muchas de las contenciones se han llevado a cabo incorrectamente y varios de los profesionales, tanto enfermeros/as como auxiliares, desconocían las indicaciones y la técnica. Este desconocimiento puede generar una experiencia traumática y lesiva, tanto para el profesional como para el paciente. Por lo tanto, el conocimiento es un factor fundamental en una contención mecánica, ya que una mala praxis genera mayor angustia en el personal enfermero. Currently there are certain times in serious and not so serious mental disorders, as well as situations of risk of falls, injuries, etc. That generate an ethical-legal obligation, on the health professional, to make use of all those therapeutic means that are available, even to carry out interventions that restrict the autonomy and freedom of the mental patient, such as mechanical containment. The use of this measure by the nursing staff, has an impact not only on a physical level, but also an emotional one. Therefore, the research objective of this study is to know how affects the use of mechanical containment at emotional level on the nursing professional. On the other hand, it analyzes the knowledge that the personnel has about the use of the measure, since it can be a cause or aggravating factor of the negative sensations generated by the management of physical restraint. The methodology used was a bibliographical review. The use of mechanical containment generates a negative emotional impact, on the nurses of the units of mental health and of geriatrics. Professionals feel fear, grief, compassion, guilt, anguish and fear of abusing the rights of patients. Carrying out a restraint can be distressing and can cause anxiety in the staff, as well as anger, sadness, worry, frustration, shock, etc. Mechanical containment is a measure in which nurses feel uncomfortable and also perceive it as a necessity for which there is no alternative. On the other hand, it has been found that many of the containments have been carried out incorrectly and several of the professionals, both nurses and auxiliaries, were unaware of the indications and technique. This ignorance can generate a traumatic and harmful experience for both the professional and the patient. Therefore, knowledge is a fundamental factor in a mechanical containment, since a bad praxis generates greater anguish in the nursing staff.
