Depressió post-part. Revisió bibliogràfica

Abstract

ANTECEDENTS La depressió post-part és una patologia que encara avui dia no sempre és ben diagnosticada i pot afectar a diferents nivells tant a la mare, com al nado i a la família sencera. OBJECTIUS Cercar quina prevalença hi ha de depressió post-part, quines són les seves causes i conseqüències, quins tractaments podem trobar i quina es la seva eficàcia, valorar escales i instruments de mesura i veure quin és el paper d’infermeria i de les llevadores per tal de prevenir aquest esdeveniment. MÊTODES Recerca bibliogràfica durant l’últim any acadèmic, on es te en compte que siguin articles d’interès, amb una publicació menor a 5 anys. En espanyol o anglès i utilitzant cercadors bibliogràfics com: Pubmed, Scielo, Cochrane i Google acadèmic. RESULTATS La causa de la depressió post-part no està ben definida i no hi ha una causa principal que la produeix sinó que hi ha diversos factors que poden influir i desencadenar-la a tenir-ho. Aquesta patologia ve acompanyada de sentiment de plors, irritabilitat, sentiment de culpa, mal humor, plors, on desencadena soledat, insomni, pèrdua de gana etc.. El paper d’infermeria i de les llevadores és molt important ja que ofereixen a la mare i a la família un recolzament personal. D’aquesta manera s’ajuda a la mare a expressar les seves pors i a intervenir en tots els dubtes, treballant la depressió post-part si escau. CONCLUSIONS La prevalença de la depressió varia entre un 10-20%, es creu que durant els pròxims anys serà la segona malaltia més freqüent, generant un problema de salut pública i econòmica. Les seves causes i conseqüències no estan ben definides ja que poden haver diversos factors els que influencien en aquest fet. El tipus de tractaments poden ser tant farmacològics com no farmacològics, trobant una amplia varietat de teràpies. S’identifiquen amb diferents escales de mesura (hetero o auto aplicada). No hi ha prou articles científics que estimin bé quin es el paper fonamental de la infermeria i les llevadores. Expliquen que han de saber de salut mental i quines poden ser les intervencions terapèutiques per tal de fer un seguiment a la mare i així poder diagnosticar abans de patir la patologia o bé fer prevenció de la mateixa. La prevalença s’estima que cada vegada anirà en augment si no es tracta o és fa una prevenció de manera adequada. Això pot crear un gran impacte en la societat tant econòmicament com en termes de salut pública.

ABSTRACK Post-delivery depression is an under-diagnosed illness in most mothers nowadays, causing distress in the mother as well as the newborn and their families. Objectives To determine which is the real prevalence of post-delivery depression, as well as defining its causes and consequences Find which treatments could be available and evaluate their efficacy and consider scales or any other mean to measure its gravity. Finally, determine whixch role do the nursery staff and the midwives play in order to prevent this illness. METHODS Bibliographic research folllowing the last academic year, taking into account the quality of the papers mentioned as follows: related to the subject, being published during the last 5 years (there is an older paper included due to its high clinical relevance), written in Spanish or English. The research was made using the following platforms: Pubmed, Scielo, Cochrane and Google academic. RESULTS The cause of post-delivery depression is not well defined yet since there is not only one main cause related to it but many factors which could lead to it. This pathology expresses itself with crying, irritability, guilt, bad humour and may trigger loneliness, insomnia, anorexia, etc. It ia estimated that its prevalence is approximately of a 10-20% and it is more prevalent in underdeveloped countries. The nursery staff and the midwives play a very important since they can offer support to the mothers and their families, which helps them express and show their fears allowing to diagnose this previous stages and helping treating depression if it occurs.

CONCLUSIONS The prevalence of depression varies between 10-20%, it is believed that in the coming years it will be the second most frequent illness, generating a public and economic health problem. Its causes and consequences are not well defined since there may be several factors that influence this fact. The type of treatments can be both pharmacological and extended to other types of therapies. They are identified with different measurement scales (hetero or self-applied). There is a lack of data in order to estimate which role do the nursery staff and midwives play, though they determine they need to have an education in mental health and have to be aware of their capabilities in order to help in the early stages of the distress or even to prevent it. The prevalence of the disease is expected to be higher if it is not treated correctly or prevented. This is of great importance in our society since it affects both economy and health.
