Evaluación de los conocimientos de enfermería sobre las Terapias de Reemplazo Renal en el paciente séptico en las diferentes unidades de Cuidados Intensivos de Can Ruti y Valle Hebrón

Abstract

INTRODUCCIÓN: La sepsis después de varias décadas continúa siendo una de las principales causas de muerte alrededor del mundo a pesar de los grandes avances médicos en su investigación. Se considera que, ante cualquier agresión, el organismo pone en marcha una respuesta inflamatoria que tiende a limitar el proceso y a conseguir la curación. En el caso de la sepsis, hablamos de una respuesta del huésped a la infección de una manera más amplia ya que, no solamente involucra las activaciones de los sistemas de respuesta pro y antinflamatorios, sino también diversas modificaciones en vías que no son inmunológicas. Por lo tanto, la sepsis se la considera una condición que surge por un desequilibrio del organismo ante una infección que pone en peligro la vida. Los profesionales de enfermería tienen un papel fundamental ya que algunos signos claros de sepsis pueden ser observables. Forman, pues, la primera línea de contacto con el paciente, por lo que su papel es de vital importancia para detectar posibles complicaciones. A lo largo del tiempo, el tratamiento para este síndrome se ha visto modificado, ampliado y mejorado. Se han incorporado nuevas terapias en la Surviving Sepsis Campaign y se han propuesto otras terapias que no acaban de ser del todo recomendadas por esta misma. Las Terapias de Reemplazo Renal (TRR) son utilizadas en el paciente séptico, para favorecer la función renal antes de que ésta sea invalida por sí misma. Algunas terapias como la Hemofiltración de Alto Volumen han sido incorporadas en este tipo de paciente, pero no hay suficientes estudios que favorezcan la recomendación de esta terapia en el paciente séptico. Las líneas de futuro van encaminadas hacia otro tipo de terapias innovadoras, pero las TRR siguen siendo ojo de investigación. OBJETIVOS: Con el presente estudio se quiere estudiar si los profesionales de enfermería de los hospitales Can Ruti y Valle Hebrón, situados en Cataluña, en las diferentes unidades de Cuidados Intensivos tienen conocimientos sobre las TRR aplicadas en el paciente séptico.

MATERIAL Y MÉTODO: Es un estudio descriptivo, observacional y transversal que pretende describir unas condiciones en un ámbito específico en un momento concreto. Mediante un cuestionario de elaboración propia se evaluaran los conocimientos de enfermería dentro del colectivo sobre las TRR y el paciente séptico en los anteriores hospitales nombrados. INTRODUCTION: Sepsis after several decades remains one of the leading causes of death in the world at the height of the great medical advances in its research. It is considered that, with an aggression, the organism sets in motion an inflammatory response that tends to limit the process and achieve healing. In the case of sepsis, it not only involves the activations of the pro and anti-inflammatory response systems, but also changes in the pathways that are not immunological. Therefore, sepsis is considered a condition that arises from an imbalance of the organism before a life-threatening infection. Nursing professionals have a fundamental role because clear signs of sepsis can be observable. They form, then, the first line of contact with the patient, so their role is of vital importance to detect possible complications. Over time, the treatment for this syndrome has been modified, expanded and improved. New therapies have been incorporated in the Survival of Sepsis Campaign and other therapies that are not related to this task have been proposed. Renal Replacement Therapies (RRT) are used in the septic patient, to favor renal function before it is invalidated by itself. Some therapies such as High Volume Hemofiltration have been incorporated in these type of patients, but there are not enough studies that favor the recommendation of this therapy in the septic patient. The lines of the future are aimed at other types of innovative therapies, but the RRT remain an eye of investigation.

OBJECTIVES: With the present study it is possible to study if the nursing professionals of the Can Ruti and Valle Hebrón hospitals, located in Catalonia, in the different Intensive Care units have knowledge about the RRT applied in the septic patient. MATERIAL AND METHOD: It is a descriptive, observational and transversal study that deals with conditions in a specific field at a specific moment. Through a self-created questionnaire, the nursing knowledge within the group about RRT and the septic patient in the last named hospitals is evaluated.
