Alteracions del còrtex somatosensorial i motor en pacients amb dolor lumbar crònic inespecífic. Valoració i tractament.

Abstract

A través d’una revisió de la bibliografia present en revistes, articles científics i llibres especialitzats, es pretén estudiar les alteracions que hi ha en el còrtex somatosensorial i motor en pacients amb dolor lumbar crònic inespecífic. Es revisa l’eficàcia del tractament somatosensorial en aquest tipus de pacients, per tal de corregir aquesta desorganització cortical, i es treballa amb control motor per acabar de corregir els dèficits motrius que presenta la zona afectada. Els resultats de la revisió indiquen que hi ha una desorganització en el còrtex somatosensorial que disminueix l’agudesa tàctil de la zona amb dolor, així com una disminució de la representació en el còrtex motor, degut a diversos aspectes no tant estudiats, però palpable a la pràctica clínica, on el control motor de la zona lumbar i abdominal es veu afectada. Finalment, es realitza un protocol d’actuació on es proposa una valoració detallada per a pacients amb dolor lumbar crònic inespecífic i l’aplicació d’un tractament somatosensorial i de control motor amb diferents tècniques específiques que milloren l’organització del còrtex i disminueixen els símptomes presents. Through a review of the literature and specialized scientific articles is intended to study the changes that occur in somatosensory and motor cortex in patients with chronic nonspecific low back pain. We review the effectiveness of somatosensory therapy to correct the cortical desorganization in this kind of patients, and work with motor control to finally correct the motor deficit presents in the affected area. The results of the review indicate that there is a desorganization in the somatosensory cortex associated with a decrease of tactile acuity in the painful area, as well as a decrease in representation in the motor cortex. Despite the lack of theoretical studies regarding this type of disorder, these results are noticed in clinical practice, where the motor control of the lumbar and abdominal zone are affected. Furthermore, there is a protocol to assist the treatment in patients who suffer from nonspecific low back pain, which refers to the application of somatosensory and motor control in order to improve the organization of the cortex and decrease the symptoms.
