Valoración y tratamiento de la epicondilitis lateral y su relación con el nervio radial

Abstract

INTRODUCCIÓN : La epicondilitis lateral se caracteriza por presentar en primera instancia una fase inflamatoria, que evolucionará si no hay una resolución de la patología a un proceso degenerativo del tendón. Es una patología muy frecuente de la extremidad superior que afecta a 1-2% de la población general, en ocupaciones que realizan tareas manuales repetitivas. El musculo comúnmente lesionado es el extensor corto radial del carpo con preferencia por el brazo dominante. METODOLOGÍA : Se realizó una búsqueda bibliografía de los últimos 10 años en las bases de datos: Pubmed, Cochcrane Plus, PEDro y CSIC, utilizando descriptores y palabras clave. Además de libros de EU Gimbernat. RESULTADOS : Tras la revisión de la literatura y en base de la mejor evidencia disponible, se elaboró una valoración y exploración exhaustiva a nivel musculoesquelético, articular y neural que diera las pruebas concluyentes para el posterior tratamiento de la epicondilitis lateral basado en la fisioterapia multimodal: Crioterapia, US, Movilización cervical, recentrajes, MWM Mulligan, liberación miofascial, Cyriax, neurodinámica Kabat, ejercicios excéntricos y de estiramiento. CONCLUSIONES : Se concluye que la afectación del nervio radial puede acompañar o ser la causa desencadenante de la epicondilitis lateral, perpetuando así el dolor y la disfunción motora. Se recomienda como tratamiento potencial la fisioterapia multimodal como una vía de acción y resolución de la patología, en especial en los casos rebeldes. Se precisa de un mayor número de estudios de alta calidad para poder generalizar la eficacia de la fisioterapia multimodal en la epicondilitis lateral. PALABRAS CLAVE : Codo de tenista. INTRODUCTION : Lateral epicondylitis is characterized by an initial inflammatory phase, which will develop, if there is no resolution of the pathology, to a degenerative process of the tendon. It is a very common disease of the upper limb that affects 1-2% of the general population, in jobs that perform repetitive manual tasks. The commonly injured muscle is the brevius radial extensor carpi with preference for the dominant arm. METHODOLOGY : A literature search of the last 10 years in the databases was performed using descriptors and keywords of: Pubmed, Cochcrane Plus Peter and CSIC. Besides books of the EU Gimbernat were consulted. RESULTS : After reviewing the literature and based on the best available evidence, it was performed an assessment and exhaustive exploration of the musculoskeletal, joint and nerve system to give conclusive evidence for the subsequent treatment of lateral epicondylitis based on multimodal physiotherapy : cryotherapy, US, cervical mobilization, recentering, MWM Mulligan, myofascial release, Cyriax, neurodynamic Kabat, eccentric and stretching exercises. CONCLUSIONS : We conclude that the involvement of the radial nerve may accompany or be the underlying cause of lateral epicondylitis, perpetuating the pain and motor dysfunction. Multimodal therapy as a course of action and resolution of the disease, especially in refractory cases is recommended as a potential treatment. It requires a greater number of high quality studies to generalize the effectiveness of multimodal physiotherapy in lateral epicondylitis. KEYWORDS : Tennis elbow. Radial nerve. Physical therapy.
