Beneficios de la implantación de un sistema aumentativo de comunicación en un paciente con PSP-PAGF

Abstract

Introducción: La Parálisis Supranuclear Progresiva (PSP), engloba a un conjunto de cuadros clínicos de entre los que se encuentra la PSP-Pure Akinesia with Gait Freezing (PSP-PAGF). A nivel de voz, su característica más destacable es la hipofonía. Esta condición, junto con otras relativas al habla, permite entrever importantes dificultades comunicativas. La necesidad de abordaje de esta situación abre la posibilidad del empleo de Sistemas Aumentativos o Alternativos de Comunicación (SAAC). Objetivo: Consiste en conocer si existen beneficios comunicativos, emocionales y relativos a la calidad de vida en un paciente con PSP-PAGF con marcada hipofonía, tras la intervención con un Sistema Aumentativo de Comunicación (SAC): un amplificador de voz. Metodología: Estudio de caso único estructurado en tres etapas: valoración inicial y toma de decisiones sobre el SAAC, implantación del mismo e intervención y valoración final. Se analiza el impacto de la intervención con el amplificador de voz, describiendo los datos extraídos de las pruebas aplicadas antes y después de su uso. Resultados: Los datos obtenidos fueron favorables en términos de comunicación y calidad de vida del paciente. Por el contrario, los aspectos emocionales no manifestaron cambios llamativos. Discusión: El empleo de SAC en pacientes con hipofonía que presentan PSP-PAGF, puede aportar beneficios comunicativos, para lo cual se necesita un tratamiento intensivo basando en objetivos funcionales. Sin embargo, se precisa de una mayor investigación que respalde los resultados obtenidos. Los beneficios emocionales se encuentran más sujetos al debate.

Introduction: Progressive Supranuclear Palsy (PSP) encloses a set of clinical pictures including Progressive Supranuclear Palsy-Pure Akinesia with Gait Freezing (PSP-PAGF). Regarding the voice, the most outstanding feature is the hypophonia. This condition, together with others related to speech, suggests important communication difficulties. The need for an approach of this situation opens the possibility to apply the Augmentative and Alternative Communication (AAC). Objective: It consists of knowing the existence of communicative and emotional benefits as well as relative to living standard in a patience with PSP-PAGF with strong hypophony, after the intervention with an Augmentative Communication System: a voice amplifier. Methodology: Study of a unique case composed of 3 stages: initial assesment and decision-making about the AAC systems, its implementation, intervention and final evaluation. The impact of the intervention is analysed with the voice amplifier, describing the data from the tests run before and after. Results: The data obteined were positive regarding patience´s communication and quality of life. On the other hand, emotional aspects did not exhibit any notable changes. Discussion: The use of augmentative communication systems in patiences with hypophonia and also represents PSP-PAGF is sable to provide communicative benefits, in which you would need and intensive treatment based on functional objetives. Howewer, it is needed a further research supporting the results obtained. The emotional benefits are subject to debate.
