El dolor lumbar i la seva relació amb la síndrome de l’intestí irritable

Abstract

Aquest treball parteix de dues simptomatologies en principi independents i va a buscar-hi una convergència. Partint d'una contextualització anatòmica de la part final de l'aparell digestiu i els sistmes relacionants, s'ha anat a buscar com s'estableix la relació entre el dolor lumbar i la síndrome de l'intestí irritable (SII). Primer s'ha fet una anàlisi individual de cadascuna d'elles i després s'han aportat les diferents hipòtesis dels mitjans pels quals es poden relacionar. La segona part del treball està enfocada a l'exploració i al tractament d'aquest dolor lumbar, utilitzant un enfocament biopsicosocial. Proposem com a pilar primàri del tractament una bona anamèsi, ben estructurada i que pari especial atenció a la relació del dolor lumbar amb el sistma digestiu i els seus possibles desordres. L'entrevista es complementarà amb una exploració física, sobre tot de la part visceral-lumbar i als sistema nerviós autònom (SNA). Per últim, segons els resultats obtinguts, passarem a tractar els teixits viscerals i fascials implicats, a regular el desequilibri autonòmic, a revisar la dieta del pacient i intentar orientar-lo cap a solucions per possibles factors psicosocials que agreugin els símptomes. This paper begins with two independent patologies and tries to look its a convergency. Starting from an anatomical contextualitzation of the final part of the digestive system and those related, we tried to find how do the lumbar pain and the Irritable Bowel Syndrome (IBS) get connected. First an individual analysis of each one of them has been done and then different hypothesis had been offered to relate them. The second part of this paper focuses on the examination and treatment of the lumbar pain, using a biopsichosocial approach. We propose as the main pillar of the treatment a good anamnesis, well structured and wich pays special attention to the relation of the lumbar pain with the digestive system and its possibles disorders. The interview will be completed with a physical exploration, mainly of the visceral and lombar parts and the autonomous nervous system. Lastly, depending on the results obtained, we will treat the visceral tissue and fascia implicated, will regulate the unbalanced nervous system, will revise patient's diet and we will try to guide him to a solution for a possible psichological factor that makes his condition worse.
