Valoració i tractament de la sensibilització central d'una pacient amb endometriosi

Abstract

La realització d’aquest treball s’ha volgut centrar en la sensibilització central (SC) existent en les pacients que pateixen endometriosi, ja que actualment encara és un àmbit que no es té gaire en compte i del que s’hauria d’investigar amb més profunditat. El dolor que pateix una persona durant molt de temps produeix un bombardeig d’estímuls al sistema nerviós causant una sensibilització perifèrica. Si aquesta es manté en el temps, acabarà produint una SC, la qual cosa provocarà canvis plàstics en el cervell, ja que aquesta ha acabat integrant el dolor dins seu. Davant d’aquest fenomen, s’haurà de realitzar un tractament específic, tenint en compte la gran importància que té la neuropedagogia del dolor que rebi la pacient. Per tant, en una pacient que pateix dolor és imprescindible explorar molt més enllà de la zona en qüestió ja que segons la recerca bibliogràfica cercada sobre la valoració i tractament fisioterapèutic òptims disponibles actualment per a la SC, s’ha pogut observar que hi ha molts factors (tant a nivell fisiològic com psicològic) i variables (nivell visceral, postural, nerviós, etc.) que poden estar influint en el seu problema i agreujar molt el quadre simptomàtic. Els músculs són més propensos a convertir-se en hiperalgèsics sota condicions recurrents de dolor visceral com la dismenorrea, i una fixació visceral no tractada produeix progressivament un bloqueig vertebral que a la vegada manté la lesió visceral, mitjançant un arc reflex, o viceversa.
