Avaluació i abordatge fisioterapèutic de la cervicàlgia i radiculopatia post fuetada cervical

Abstract

INTRODUCCIÓ: La fuetada cervical, sobretot la derivada dels accidents de trànsit és una de les patologies amb major prevalença en els darrers anys. Hi ha més de 60 casos nous per cada 100.000 habitants l’any a Espanya, i es contempla que anirà a l’alça per l’increment de la circulació i l’augment de l’ús de les noves tecnologies durant la conducció que magnifiquen el risc de col·lisió, ja que suposen una distracció perillosa. El terme de fuetada cervical correspon a l’excés d’acceleració-desacceleració de forma brusca i sobtada del cap i coll. Aquesta força d’inèrcia provoca múltiples simptomatologies als pacients que comprenen des de cefalees i cervicàlgies fins a les alteracions motores, sensorials i nervioses. OBJECTIUS: L’objectiu principal del nostre treball és avaluar el millor diagnòstic i tractament de fisioteràpia pel dolor cervical causat per una fuetada cervical. RESULTATS – PLA D’ACTUACIÓ: Hi ha diferents tipus de valoració per aquesta patologia, des de palpació a l’avaluació muscular, passant per la valoració del dolor i els seus aspectes psicològics fins a proves complementàries com l’ecografia. S’ha comprovat que realitzant un bon control motor juntament amb una teràpia psicològica i un tractament conductual, on també s’introdueix la teràpia manual activa, donen uns resultats més eficients i menys invasius, aconseguint una major disminució del dolor i una readaptació precoç al món laboral, en comparació al tractament habitual. CONCLUSIONS: El treball està enfocat al diagnòstic i tractament de la fuetada cervical subaguda i per això, volem proposar un abordatge del pla terapèutic més eficaç i complementari al convencional. INTRODUCTION: Whiplash injury, especially when derived from traffic accidents, is one of the most prevalent pathologies in recent years. There are more than 60 new cases per every 100,000 inhabitants per year in Spain, and it’s contemplated that this will increase due to the intensification of circulation and use of new technologies during driving that magnify the risk of collision, since they involve a dangerous distraction. The term of whiplash injury corresponds to the excess of sudden acceleration-deceleration of the head and neck. This force of inertia causes multiple symptomatology to patients that include from headache and neck pain to motor, sensory and nervous alterations. OBJECTIVES: The main objective of our work is to evaluate the best diagnosis and treatment of physiotherapy for neck pain from a whiplash injury. RESULTS - PLAN OF ACTION: There are different types of assessment for this pathology- from palpation to muscle evaluation, through pain assessment and its psychological aspects to complementary tests such as ultrasound. It has been shown that performing good motor control along with a psychological therapy and behavioral treatment, where active manual therapy is also introduced, give more efficient and less invasive results, achieving a greater decrease in pain and early readaptation at Labor market, compared to usual treatment. CONCLUSIONS: This work is focused on the diagnosis and treatment of subacute whip injury and for this reason, we would like to propose a more effective and complementary treatment to the conventional therapeutic approach.
