El codo de tenista: influencia del nervio radial, valoración y tratamiento

Abstract

INTRODUCCIÓN: El codo de tenista (CT) está relacionado con los músculos del epicóndilo lateral, comúnmente con el extensor radial corto del carpo y durante la mediana edad, sus síntomas duran entre 6-24 meses. Asociado a movimientos repetitivos de pronosupinación y extensión del antebrazo o con carga. El nervio radial (NR) atraviesa la musculatura del epicóndilo lateral, el atrapamiento en el túnel radial se diagnostica muchas veces con las mismas pruebas que el CT, y su mecanismo lesional es parecido, por lo que cuesta pensar en que son patologías independientes. OBJETIVOS: Poder diagnosticar efectivamente ambas patologías, demostrar mediante la búsqueda bibliográfica que no son entidades clínicas separadas y utilizar el tratamiento más efectivo. RESULTADOS – PLAN DE ACTUACIÓN: En el 41% de CT el test de provocación del NR es positivo. Los puntos gatillo miofasciales (PGM) se encuentran activos en los músculos del epicóndilo lateral en un gran porcentaje. El questionario Patient Rated Tennis Elbow Evaluation (PRTEE) engloba 3 grupos de tratamiento: bajo riesgo, riesgo moderado y alto riesgo (incluyendo aquí la sensibilización central). El tratamiento conservador de forma multimodal, mejora de forma significativa en comparación al placebo. CONCLUSIONES: El tratamiento debe enfocarse hacia las dos patologías. La evolución es mejor cuando el paciente se involucra de forma activa, y si se trata desde una perspectiva biopsicosocial. El tratamiento de PGM y de neurodinámica son esenciales por su afectación, combinado con ejercicios de fortalecimiento y terapia manual. La PRTEE será el principal indicador sobre si el tratamiento es adecuado y efectivo. INTRODUCTION: The tennis elbow (TE) is related to the muscles of the lateral epicondyle, commonly with the short radial extensor of the carpus and during middle age, its symptoms last between 6-24 months. Associated with repetitive movements of prono-supination and extension of the forearm or with load. The radial nerve passes through the lateral epicondyle musculature, entrapment in the radial tunnel is often diagnosed with the same tests as the tennis elbow, and its mechanism of injury is similar, so it is difficult to think that they are independent pathologies. OBJECTIVES: To be able to effectively diagnose both pathologies, to demonstrate through the bibliographic search that they are not separate clinical entities and to use the most effective treatment. RESULTS - ACTION PLAN: In 41% of TE the radial nerve provocation test is positive. The trigger points (TP) are active in the muscles of the lateral epicondyle in a large percentage. The Patent Rated Tennis Evaluation (PRTEE) questionnaire encompasses 3 treatment groups: low risk, moderate risk and high risk (including central sensitization here). Conservative multimodal treatment improves significantly compared to placebo. CONCLUSIONS: Treatment should focus on the two pathologies. The evolution is better when the patient is actively involved, and if it is treated from a biopsychosocial perspective. The treatment of TP and neurodynamics are essential for its affectation, combined with strengthening exercises and manual therapy. The PRTEE will be the main indicator on whether the treatment is adequate and effective.
