Tractament del control postural amb LSVT BIG a la malaltia de Parkinson

Abstract

RESUM. INTRODUCCIÓ: La malaltia de Parkinson és una malaltia neurodegenerativa que cursa amb característiques motores i no motores i que condueix a una progressiva discapacitat funcional. Actualment, el tractament està basat en fàrmacs que augmenten la dopamina, però no existeixen tractaments que aturin el progrés de la malaltia. A més, els estudis demostren l’efectivitat de la fisioteràpia com a tècnica complementària,amb teràpies com Lee Silverman Voice Treatment –BIG (LSVT BIG). OBJECTIUS: L’objectiu principal d’aquestestudiés desenvolupar un tractament fisioterapèutic anomenat LSVT BIG per a lamillora del control postural. RESULTATS: Per al diagnòstic dels símptomes s’ha realitzat una anamnesi als pacients i s’han utilitzat diverses escales de valoració. Segons els resultats previsibles de la valoració es distingeixen tres grups de pacients, de menys a més afectació:grup I (estadis I i II de Hoehn i Yahr), grup II (estadis III iIV de Hoehn i Yahr)igrup III (estadi V de Hoehn i Yahr). El pla terapèutic basat en els exercicis LSVT Big és específic per a cada grup i segueix les pautes necessàries per aconseguir la millora funcional del pacient. CONCLUSIONS: Aquest pla diagnòstic permet determinar l’estat delspacients i categoritzar-los en grups, fet que ajuda a individualitzar el tractament. La teràpia LSVT BIG és una eina de treball útil per als professionals de la salut corresponents, ja que proporciona una millora dels dèficitsmotorsdels pacients,augmentael control postural i aporta beneficis ala qualitat de vida. ABSTRACT. INTRODUCTION: Parkinson’s disease is a neurodegenerative disease that involves motors and non-motors features and evolves into a progressive functional disability. Currently, its treatment is based on drugs that increase dopamine, but there are no treatments to stop the progression of the disease. In addition, studies demonstrate the effectiveness of physiotherapy as a complementary technique, with therapies such as Lee Silverman Voice Treatment –BIG (LSVT BIG) . OBJECTIVES: The main objective of this study is develop a physiotherapeutic treatment called LSVT BIGin order to enhance the postural control. RESULTS: For the diagnosis of symptoms, an anamnesis has been done to the patients and various valuation scales have been used. According to the foreseeable results of the valuation, three groups of patients are distinguished, from less to more affectation: group I (stagesI and II of Hoehn and Yahr), group II (stages III and IV of Hoehn and Yahr) and group III (stage V of Hoehn and Yahr). The therapeutic plan based on the LSVT BIG exercises is specific to each group and follows the guidelines necessary to achieve the functional improvement of the patient. CONCLUSIONS: This diagnostic plan allows the determination of patient status and categorize them in groups, helping to individualize treatment. LSVT BIG therapy is a useful working tool for the corresponding health professionals, because it provides an improvement in patient’s motor deficits, increases postural control and brings benefits to the quality of life.
