Evaluación de la tendinopatía medial del codo: diagnóstico, indicaciones y tratamiento fisioterapéutico con ondas de choque

Abstract

RESUMEN. INTRODUCCIÓN: La tendinopatía medial del codo o “codo de golfista” es una patología multifactorial de la extremidad superior que se caracteriza por dolor en la región musculotendinosa del epicóndilo medial.Se cree que la causa principal es la combinación de movimientos repetitivos de los músculos flexopronadores junto con una estabilización forzada en valgo del codo.OBJETIVOS: Ser capaces de realizar un diagnóstico diferencial adecuado de la gran variedad de patologías que pueden dar dolor en el epicóndilo medial del codo. Proponer la terapia mediante ondas de choque extracorpóreas como parte del tratamiento fisioterapeútico. RESULTADOS: Para poder diagnosticar de manera correcta la tendinopatía medial del codo es necesario descartar otras entidades clínicas. El tratamiento de primera línea de la tendinopatía medial del codo es conservador.La terapia de ondas de choque está siendo estudiada como parte del tratamiento, pero se requieren más estudios para conocer su adecuado procedimiento. CONCLUSIONES: Realizar un buen diagnóstico diferencial sobre las patologías que dan dolor en el epicónidlo medial para poder individualizar su posterior tratamiento. La terapia de ondas de choque parece ser efectiva en las fases aguda y subaguda de la tendinopatía medial del codo. A su vez, es capaz de prevenir la cronicidad de esta patología. ABSTRACT. INTRODUCTION: Medial tendinopathy of the elbow or "golfer's elbow" is a multifactorial pathology of the upper extremity characterized by pain in the musculotendinose region of the medial epicondyle. It is believed that the main cause is the combination of repetitive movements of the flexo pronators muscles along with forced valgus stabilization of the elbow.OBJECTIVES: The aim of this review is to be able to make an adequate differential diagnosis of the wide variety of pathologies that can cause pain in the medial epicondyle of the elbow. At the same time, it also aims to propose therapy based on extracorporeal shock waves as part of the physiotherapist treatment. RESULTS: In order to diagnose the medial tendinopathy of the elbow correctly, it is necessary to discard other clinical entities. The first-line treatment of medial tendinopathy of the elbow is conservative. On the other hand, it has been found that shock wave therapy is being studied as part of the treatment at present, but more studies are required to know its suitable procedure. CONCLUSIONS: It is primordial to carry on a good differential diagnosis on pathologies that can cause pain in the medial epicondyle, in order to individualize its subsequent treatment. Shock wave therapy appears to be effective in the acute and subacute phases of medial tendinopathy of the elbow. In turn, it is able to prevent the chronicity of this pathology.
