Cuidados de enfermería al paciente con depresión mayor desde una perspectiva eudaimónica

Abstract

INTRODUCCIÓN: A nivel mundial, aproximadamente 350 millones de personas sufren de depresión mayor. Es la principal causa mundial de discapacidad y se la relaciona con el 90% de los suicidios consumados. En la actualidad existe una pobre respuesta a esta problemática. Hay diferentes enfoques sobre cuál es el origen de la depresión, que a la vez, defienden abordajes diferentes y difíciles de consensuar, y excluyen y dejan al margen otros determinantes de la salud mental como las presiones sociales y del medio ambiente. Además, la línea de tratamiento parece ir encaminada exclusivamente al tratamiento con “antidepresivos” (por varios alicientes), pese a las carencias y dificultades que muestran, y a que se les relaciona con altas tasas de suicidio. Por ello se hace necesario hallar otra forma de cuidar la salud mental que se base en un modelo más humano y real, que deposite una mayor confianza en las personas para salir adelante a pesar de las adversidades y facilite la recuperación y el crecimiento personal. OBJETIVO: Diseñar una propuesta de intervención de enfermería basada en la psicología positiva que pueda ayudar a mejorar la salud mental y la calidad de vida de las personas con depresión mayor. METODOLOGÍA: Se realizó una búsqueda bibliográfica y análisis de la literatura científica sobre la depresión mayor, la salud mental positiva y la psicología positiva durante los meses de septiembre de 2020 a mayo de 2021. Para la búsqueda se establecieron unos criterios de búsqueda y se utilizaron bases de datos y otras fuentes de información como: Medline (PubMed), SciELO,ScienceDirect, Google Académico, WHO, WFMH, NIMH, CSME, AEESME…. RESULTADOS: Como resultado se presenta una propuesta de intervención que consiste en una adaptación del Decálogo de Salud Mental Positiva elaborado por la Dra. María Teresa Lluch-Canut en 2011. Esta versión va dirigida específicamente a pacientes con depresión mayor y tiene un enfoque derivado de la felicidad. CONCLUSIONES: Es indispensable dejar de favorecer a la teoría biológica y al abordaje meramente farmacológico de la depresión mayor, que resulta simplista e inocuo y proviene de modelos patógenos de salud mental. Y por ende, subyace la necesidad de abrir paso a modelos salutógenos que se enfoquen en capacitar a la persona en estilos de vida más saludables (psíquicos, físicos, emocionales, familiares, sociales, laborales, ambientales…) que favorezcan su salud mental positiva y le enseñen a saber ser y estar en la vida. In addition, the line of treatment seems to be aimed exclusively at treatment with "antidepressants" (for various reasons), despite the deficiencies and difficulties they show, and the fact that they are related to high suicide rates. For this reason, it is necessary to find another way of caring for mental health that is based on a more human and real model, which places greater trust in people to get ahead despite adversity and facilitates recovery and personal growth. OBJECTIVE: Design a proposal for a nursing intervention based on positive psychology that can help improve the mental health and quality of life of people with major depression. METHOD: A bibliographic search and analysis of the scientific literature on major depression, positive mental health and positive psychology was carried out during the months of September 2020 to May 2021. For the search, search criteria were established and databases were used. data and other sources of information such as: Medline (PubMed), SciELO, ScienceDirect, Google Scholar, WHO, WFMH, NIMH, CSME, AEESME…. RESULTS: As a result, an intervention proposal is presented that consists of an adaptation of the Positive Mental Health Decalogue prepared by Dr. María Teresa Lluch-Canut in 2011. This version is specifically aimed at patients with major depression and has an approach derived from happiness. CONCLUSIONS: It is essential to stop favoring the biological theory and the merely pharmacological approach to major depression, which is simplistic and innocuous and comes from pathogenic models of mental health. And therefore, underlies the need to open the way to salutogenic models that focus on training the person in healthier lifestyles (mental, physical, emotional, family, social,work, environmental ...) that favor their positive mental health and teach you to know how to be and be in life.
