Intervención cognitiva con familias en pacientes con enfermedad de Alzheimer: protocolo de estudio de ensayo clínico

Abstract

La Enfermedad de Alzheimer (EA) es una enfermedad neurodegenerativa caracterizada principalmente por la pérdida de memoria episódica, la afectación del lenguaje y de las funciones ejecutivas. Este deterioro cognitivo conlleva dependencia en los cuidados que suele generar un estrés o sobrecarga psicológica, mayormente en el cuidador principal. Dado que no existe un tratamiento definitivo para la EA, se realizan tratamientos no farmacológicos que pueden mejorar la calidad de vida. En este trabajo se realiza un estudio en base a la terapia no farmacológica de rehabilitación (RHB) cognitiva incluyendo a la familia como co-terapeuta. Se pretende comprobar si esta RHB frente a la tradicional sin la figura del familiar, permite mejorar o mantener las capacidades cognitivas (lenguaje, memoria, funciones ejecutivas y praxias) de la persona con EA y reducir la sobrecarga del cuidador. Consistirá en una RHB de tres meses dirigida a 24 pacientes de entre 60 y 80 años que presenten EA. Se diferenciarán dos grupos de estudio de 12 participantes cada uno, contando con la figura del familiar como co-terapeuta en un grupo con el fin de comparar los resultados tras finalizar el estudio. Se espera obtener beneficios en las personas con EA en el desempeño de actividades rutinarias que requieren habilidades cognitivas como una comunicación funcional y una adaptación eficaz al entorno. En este proceso, no se atiende solo al paciente, sino que se presta atención al familiar, aportando conocimientos y herramientas que pueden generalizarse en su día a día, pudiendo disminuir así la sobrecarga y el estrés del familiar.

Alzheimer's disease (AD) is a neurodegenerative disease characterized mainly by episodic memory loss, language impairment and impaired executive functions. This cognitive impairment leads to caregiver dependence that usually generates stress or psychological overload, mostly in the primary caregiver. Since there is no known definitive treatment for AD, non-pharmacological treatments can improve quality of life. In this work, a study is carried out based on non-pharmacological cognitive rehabilitation therapy (RHB) including the family as co-therapist. The aim of this study is to determine whether this BRT, as opposed to traditional therapy without the family member, can improve or maintain the cognitive abilities (language, memory, executive functions and praxis) of the person with AD and reduce caregiver overload. It will consist of a three-month RHB aimed at 24 patients between 60 and 80 years of age with AD. Two study groups of 12 participants each will be differentiated, with the figure of the family member as co-therapist in one group in order to compare the results after the end of the study. It is expected to obtain benefits in people with AD in the performance of routine activities that require cognitive skills such as functional communication and effective adaptation to the environment. In this process, attention is not only paid to the patient, but also to the family member, providing knowledge and tools that can be generalized in their daily life, thus reducing the overload and stress of the family member.
