Impacte del nivell formatiu d'infermeria en pacient crític, urgent i emergent

Abstract

RESUM: Des de fa temps, diverses societats i associacions d’infermeria reclamen que es reguli i es formalitzi l’especialitat en cuidats crítics i en cuidats que requereix el pacient que es troba en situacions d’urgència i emergència. I és que són molts els elements diferencials que es donen en serveis que tracten aquest tipus de pacient. Les valoracions, orientacions diagnòstiques, interpretacions i intervencions, el maneig de recursos tecnològics i maquinària que s’utilitza, i un llarg etcètera, comporta que aquest tipus de pacient, requereixi d’uns coneixements avançats més enllà dels continguts que ofereix el grau d’infermeria. Així ho reflecteixen la gran quantitat de màsters, post graus i títols propis que existeixen en relació a aquesta formació específica vinculada al pacient crític, urgent i emergent. D’altra banda, també ho constaten documents oficials com el que fa públic el Ministeri de Sanitat i Política Social, els quals esmenta que serveis com poden ser les Unitats de Cures Intensives -un dels serveis més paradigmàtics pel que fa al pacient crític-, requereix de professionals d’infermeria altament qualificats i que a més a més, explícitament menciona que és necessari fomentar la seva especialitat. Així doncs, serveis com urgències i emergències, tant intra hospitalàries com extra hospitalàries, i serveis com són les Unitats de Cures Intensives, són espais paradigmàtics on es realitzen moltes valoracions i reavaluacions, orientacions diagnòstiques, intervencions amb les seves corresponents prioritzacions i totes les cures que requereix un pacient crític que poden requerir d’uns coneixements específics que garanteixin una bona pràctica i en definitiva, que vetllin per la seguretat clínica del pacient. En les especialitats d’infermeria existents aprovades en el Reial Decret 450/2005, no es contempla que cap d’elles es focalitzi en el pacient i les cures en crítics. Tot i així, sí que es menciona i es categoritza l’especialitat d’infermeria medico quirúrgica, encara per aprobar el seu contingut formatiu i molt criticada per la seva poca especificitat, ja que inclou un gran volum de continguts i serveis. Tenint en compte la necessitat real i el reconeixement que existeix per part de moltes societats, associacions i personal docent, assistencial i gestor d’aquesta formació ampliada que requereix el personal d’infermeria que treballa en unitats on podem trobar el pacient crític, aquest estudi pretén avaluar si les persones que tenen uns coneixements a nivell de màster entorn el pacient crític, tenen un impacte beneficiós en l’assistència, pràctica i seguretat clínica del pacient en comparació amb les persones que no tenen aquesta formació específica.

ABSTRACT: From time to time, lots of nursing societies and associations, are claiming the specialty in critical, urgency and emergency care. There are a lot of different elements in this kind of services where we find the critical patient. The ratings, the diagnostic guidelines, the interpretations, the clinical and nurse interventions, the technological resources and the machines that the nurses use in services like Intensive Care Units or Urgency and Emergency service, need advanced and special knowledge that can not be learned only with the current nurse degree. The plural offer of master’s degree that we can find about critical, urgent and emergency patient, reflects this situation. On the other hand, also we find some oficial documents such as the one published by the Spanish Ministery of Healt and Social Policy, wich recognize the requirement to have special and high qualified nurses who works in this kind of services. This oficial document also says that is necesary to foment this specialization. Urgency and Emergency services or the Critical Care Units, are the paradigmatic services where the nurses performs lot of clinical ratings, diagnostic guidelines and nurse interventions with a good prioritization that can require a specific knowledge about the critical patient for definitively, guarantee a good and safety clinical practice. The nursing specialities approved in Royal Decree 450/2005, they do not include the critical, urgenecy and emergency patient and cares. However, the speciality of medical-surgical nursing, who includes this kind of services, is also in standby because it includes a lot of content and services. For this reason, the speciality is also in standby and has not been approved. Also it has been criticized for his nonspecificity, his large volumen of content and for lots of different services that includes. The real need and the recongition by lots of many professional societies and teaching staff of the speciality and the special, concret and higher knowledge that needs the nurses who work in this kind of service where whe can find the critical patient, encourages to do this study. This pretends to prove that nurses who have a higher knowledge with a master degree of critical patient, works with better practice and clinical safety than nurses who do not have this specific knowledge.
