Intervenció precoç en el desenvolupament motriu gruixut dels prematurs dels 0 als 18 mesos

Abstract

RESUM : INTRODUCCIÓ: L’augment de la supervivència dels nadons prematurs (nascuts <37 setmanes de gestació (SG)), ha suposat un augment de les complicacions a llarg termini, com és el retard en el desenvolupament motriu (DM). És per això que és primordial realitzar una intervenció precoç de fisioteràpia, per tal de prevenir i millorar les habilitats motrius dels prematurs. OBJECTIUS: Determinar l’efectivitat d’una intervenció precoç en el DM gruixut dels prematurs, comparar aquest DM entre els grups de prematurs i identificar en quina edat apareixen certes habilitats motrius. DISSENY: Estudi no controlat, d’abans i després de 6 nadons prematurs (0-18 mesos) amb un retard en el DM gruixut. Se’ls hi ha realitzat una intervenció precoç, 1 cop a la setmana (45 minuts) durant 8 sessions al CDIAP d’Igualada. MEDICIONS PRINCIPALS: S’ha realitzat un anàlisis estadístic, mitjançant l’Excel i la T-student de les variables del DM gruixut, l’edat d’adquisició i el DM gruixut dels diferents grups de prematurs, mitjançant l’escala AIMS i les dades prèvies obtingudes pel CDIAP. RESULTATS: No s’han obtingut diferències significatives (p>0,05) entre els percentils inicials i finals del DM gruixut, ni diferències significatives entre els grups de prematurs, a excepció del grup de molt prematurs (p=0,04), com tampoc en les edats d’adquisició de les habilitats motrius. CONCLUSIONS: Aquests resultats, podrien indicar que aquesta intervenció no es suficient per millorar els percentils finals de l’AIMS, tot i millorar-ne el DM gruixut i la seva puntuació. Calen més investigacions amb una mostra i amb un període d’intervenció més llarg i intensiu amb l’objectiu de extreure conclusions més sòlides. ABSTRACT: INTRODUCTION: The increased survival of preterm infants (born <37 weeks gestation) has led to an upsurge in long-term complications, such as delayed motor development (MD). This is why early physiotherapy intervention is essential to prevent and improve motor skills in preterm infants. OBJECTIVES: The main objectives are determining the effectiveness of early intervention in gross MD in preterm infants, comparing this MD between groups of preterm infants and identifying at what age certain motor skills appear. DESIGN: 6 premature infants (0-18 months) with a delay in gross MD were analysed and an early intervention was carried out once a week (45 minutes) during 8 sessions at the CDIAP in Igualada. MAIN MEASUREMENTS: A statistical analysis was carried out using Excel and T-student of the variables gross MD, age of acquisition and gross MD of the different groups of preterm infants, using the AIMS scale and the previous data obtained by the CDIAP. RESULTS: No significant differences (p>0.05) were obtained between the initial and final percentiles of the gross MD, nor significant differences between the groups of preterm infants, with the exception of the very preterm group (p = 0.04), nor in the ages of acquisition of motor skills. CONCLUSIONS: These results might indicate that this intervention is not sufficient to improve final AIMS percentiles, although improving gross MD and its score. Further research with a larger sample size and with a longer and more intensive intervention period is needed in order to draw more solid conclusions.
