Eficacia de la Terapia Acuática sobre la función motora gruesa en niños con Parálisis Cerebral con GMFCS I-IV

Abstract

RESUMEN : INTRODUCCIÓN: Dos de las actividades motoras gruesas más importantes afectadas por la parálisis cerebral (PC) son el equilibrio y la marcha. La terapia acuática es una intervención sugerida para los niños con PC. OBJETIVOS: Evaluar los efectos de la terapia acuática mediante el concepto Halliwick sobre las funciones motoras gruesas y el equilibrio en niños de 3 a 21 años con PC (GMFCS I-IV) y comprobar si los efectos perduran en el tiempo. DISEÑO: Estudio piloto de intervención antes-después y no controlado mediante el concepto Halliwick en niños de 3 a 21 años con PC (GMFCS I-IV), sin miedo al agua y con consentimiento informado firmado. MEDICIONES PRINCIPALES: Se valoraron las funciones motoras gruesas y el equilibrio. Se realizó un análisis estadístico univariante mediante test estadísticos paramétricos para variables numéricas con distribución normal: t Student. RESULTADOS: Las funciones motoras gruesas y el equilibrio mejoraron significativamente (p valor = 0,00046; p valor = 0,00302, respectivamente). Los resultados respaldan que el enfoque combinado terrestre y acuático es el más apropiado. Además, en corto plazo la intervención podría dar resultados óptimos. CONCLUSIONES: Parece ser que la terapia acuática mediante el concepto Halliwick acompañada de fisioterapia habitual mejoran las funciones motoras gruesas y el equilibrio en niños de 7 a 16 años con PC (GMFCS I-IV). Se sugieren realizar ensayos controlados y aleatorizados, de simple ciego, con grupo control, con una muestra grande y homogénea. ABSTRACT : INTRODUCTION: Two of the most important gross motor activities affected by cerebral palsy (CP) are balance and gait. Aquatic therapy is a suggested intervention for children with CP. OBJECTIVES: To assess the effects of aquatic therapy using the Halliwick concept on gross motor functions and balance in children aged 3 to 21 years with CP (GMFCS I-IV) and to see if the effects persist over time. DESIGN: Pilot study of intervention before and after and not controlled by the Halliwick concept in children from 3 to 21 years old with CP (GMFCS I-IV), without fear of water and with signed informed consent. MAIN MEASUREMENTS: Gross motor functions and balance were assessed. A univariate statistical analysis was performed using parametric statistical tests for numerical variables with normal distribution: t Student. RESULTS: Gross motor functions and balance improved significantly (p value = 0.00046; p value = 0.00302, respectively). The results support that the combined terrestrial and aquatic approach is the most appropriate. In addition, in the short term the intervention could give optimal results. CONCLUSIONS: It appears that aquatic therapy using the Halliwick concept accompanied by routine physical therapy improves gross motor functions and balance in children aged 7 to 16 years with CP (GMFCS I-IV). Randomized, single-blind, controlled trials with a large, homogeneous sample are suggested.
