Efecto del programa NOPPI de intervención temprana sobre el desarrollo motor en grandes prematuros

Abstract

RESUMEN : INTRODUCCIÓN: la prematuridad aumenta el riesgo de sufrir problemas motores, cognitivos y comportamentales. La intervención precoz es una opción de tratamiento para evitar la aparición de secuelas derivadas de la prematuridad. Uno de los programas destacados es el Norwegian Physiotherapy Study in Preterm Infants (NOPPI), una terapia intensiva que pretende tener efectos positivos sobre el desarrollo motor. OBJETIVOS: investigar los efectos a corto plazo del programa NOPPI sobre el desarrollo motor y los movimientos generales (GMs) y determinar la adherencia por parte de los cuidadores. DISEÑO: estudio científico, de intervención, no controlado, de tipo antes-después. Realizado en grandes prematuros médicamente estables en UCIN, sin malformaciones ni síndromes. MEDICIONES PRINCIPALES: desarrollo motor pre y post-intervención (escala TIMP, T-Student), los GMs pre- y post-intervención (escala GMA, Chi-Cuadrado y GMOS, T-Student) y la adherencia al tratamiento (porcentaje de sesiones). RESULTADOS ESPERADOS: se esperaba encontrar mejoras estadísticamente significativas en el desarrollo motor y la adherencia al tratamiento y una correlación positiva entre ambas variables. Ello significaría que el programa es efectivo para la mejora del desempeño motor y que los resultados sobre el mismo serían directamente proporcionales a la dosis recibida. No se esperaba encontrar resultados estadísticamente significativos en los GMs. CONCLUSIONES: obteniéndose los resultados esperados, existirían indicios para pensar que el programa NOPPI tiene efectos positivos sobre el desarrollo motor de grandes prematuros, pero no se podrían extraer conclusiones extrapolables a la totalidad de la población debido al diseño del estudio. En estudios futuros debería incluirse una muestra mayor, grupos intervención y control y un seguimiento. ABSTRACT : INTRODUCTION: prematurity increases the risk of suffering motor, cognitive and behavioral problems. Early intervention is a treatment option to avoid the appearance of sequelae derived from prematurity. One of the programs available is the Norwegian Physiotherapy Study in Preterm Infants (NOPPI), an intensive therapy that claims to have positive effects on motor development. OBJECTIVES: to investigate the short-term effects of the NOPPI program on motor development and general movements (GMs) and to determine adherence to the treatment by caregivers. DESIGN: scientific, interventional, uncontrolled, before-after study. It should be performed in medically stable very preterm babies in the NICU, without malformations nor syndromes. OUTCOME MEASURES: motor development (pre- and post-intervention, assessed with TIMP score, Student’s-T), GMs (pre- and post-intervention, assessed with GMA, Chi-Square, and GMOS, Student’s-T) and adherence to treatment (percentage of sessions). EXPECTED OUTCOMES: it was expected to find statistically significant improvements in motor development and adherence to treatment and a positive correlation between both variables. This would mean that the program is effective in improving motor performance and that its results would be directly proportional to the dose received. It was not expected to find statistically significant results in the GMs. CONCLUSIONS: should the expected results be obtained, there would be indications to think that the NOPPI program has positive effects on the motor development of large preterm infants, but conclusions that can be extrapolated to the entire population could not be drawn due to the design of the study. Future studies should include a larger sample, intervention and control groups, and follow-up.
