Avaluació i abordatge de fisioteràpia respiratòria en el malalt neuromuscular amb Esclerosi Múltiple

Abstract

RESUM : INTRODUCCIÓ: L’esclerosi Múltiple (EM) és una malaltia crònica inflamatòria desmielinitzant del Sistema Nerviós Central amb una evolució impredictible que cursa amb afectació i simptomatologia molt variant a nivell de tot el cos. Durant el curs de la malaltia són freqüents les complicacions respiratòries, les quals incapaciten molt al pacient limitant les activitats de la vida diària. La rehabilitació respiratòria es proposa com un abordatge preventiu i terapèutic, complementari al tractament convencional per prevenir i millorar la simptomatologia establerta a través de l’exercici terapèutic i de diferents tècniques de rehabilitació respiratòria. OBJECTIUS: Investigar i proposar una avaluació del pacient amb EM amb simptomatologia respiratòria, per realitzar un abordatge terapèutic basat en la rehabilitació respiratòria, per prevenir i millorar possibles complicacions respiratòries i millorar la qualitat de vida. RESULTATS: Les actuacions diagnòstiques que s’han portat a terme per poder catalogar els pacients consisteixen en una anamnesi general i específica per conèixer més al pacient, una exploració i una valoració tant respiratòria com funcional. A partir d’aquestes actuacions diagnòstiques hem pogut catalogar els pacients en 3 perfils diferents: pacients amb EM lleu sense afectació respiratòria, EM lleu/moderada amb afectació de la fase espiratòria i EM lleu/moderada amb afectació de la fase inspiratòria. Les pautes d’actuació terapèutica es basen en la rehabilitació respiratòria, incloent tècniques específiques de fisioteràpia respiratòria i pautes d’exercici físic. CONCLUSIONS: Mitjançant aquesta avaluació i abordatge de fisioteràpia respiratòria s'aconsegueix diagnosticar, classificar i tractar els pacients amb EM amb simptomatologia respiratòria. ABSTRACT : INTRODUCTION. Multiple Sclerosis (MS) is a chronic inflammatory demyelinating disease of the Central Nervous System with an unpredictable evolution that runs with varying impairment and symptomatology at entire body level. Throughout the course of the disease, respiratory complications are very prevalent, which incapacitates the patient in daily life activities. Respiratory rehabilitation is suggested as a preventive and therapeutic approach, complementary to the conventional treatment, in order to prevent and improve the established symptomatology through therapeutic exercise and several techniques of respiratory rehabilitation. OBJECTIVES:: Investigate and suggest and evaluation of the MS patient with respiratory symptomatology, to conduct a therapeutic approach based in respiratory rehabilitation, to prevent and improve possible respiratory complications and to improve the quality of life. RESULTS: The diagnostic interventions that have been carried out in order to catalog the patients consist in a general and specific anamnesis to know the patient and a respiratory and functional exploration and evaluation. Through these diagnostic procedures, we have catalogued the patients in 3 different profiles: mild MS patients without respiratory impairment, mild/moderate MS patients with spiratory phase affect and mild/moderate MS patients with inspiratory phase affect. The therapeutic guidelines are based on respiratory rehabilitation, including from specific techniques of respiratory physiotherapy and guidelines of physical exercise. CONCLUSIONS: Through this evaluation and respiratory physiotherapy approach it is possible to diagnose, classify and treat MS patients with respiratory symptomatology.
