Eficacia del abordaje osteopático estructural en pacientes que usan plantillas ortopédicas como tratamiento de la fasciopatía plantar sintomática

Abstract

RESUMEN : INTRODUCCIÓN: La fasciopatía plantar carece de un consenso definido sobre su abordaje. Su alta incidencia y el dolor limitante en la actividad, hacen que la comprensión del diagnóstico y las recomendaciones actuales basadas en la evidencia para el tratamiento, sean de gran importancia para el clínico. La combinación de varias técnicas parece ser más efectiva que cualquier técnica utilizada de forma aislada. OBJETIVOS: Evaluar los efectos del abordaje osteopático estructural en pacientes que usan plantillas ortopédicas como tratamiento de la fasciopatía plantar sintomática, mediante el estudio de los cambios en el dolor, movilidad y funcionalidad del miembro inferior. DISEÑO: Estudio piloto, analítico, longitudinal, prospectivo, controlado, aleatorizado a simple ciego. La muestra es de 6 pacientes del “Centre de fisioteràpia Teresa Rodríguez”, con diagnóstico clínico de fasciopatía plantar y portadores de plantillas ortopédicas. MEDICIONES PRINCIPALES: La sensación subjetiva de dolor, el rango articular activo de la flexión dorsal de tobillo y la funcionalidad del miembro inferior. RESULTADOS: No se obtiene un cambio estadísticamente significativo en la evolución del dolor, a pesar de existir una diferencia de 1,67 puntos de media en la escala EVA (P<0,296) del grupo experimental, en comparación con el grupo control. CONCLUSIONES: No se puede constatar que el abordaje osteopático estructural reduzca los valores subjetivos de dolor en pacientes con fasciopatía plantar sintomática. A pesar de la poca significancia estadística obtenida en este estudio, se han expresado valores promedios de cambio superiores en el grupo experimental con respecto al grupo control en las variables de dolor y funcionalidad. ABSTRACT : INTRODUCTION: The plantar fasciopathy lacks a defined consensus on its approach. Its high incidence and limiting pain in the activity make the understanding of the diagnosis and the current recommendations based on the evidence for the treatment of great importance for the clinician. The combination of several techniques seems to be more effective than any technique used in isolation. OBJECTIVES: To evaluate the effects of the structural osteopathic approach in patients using orthopedic insoles as treatment for symptomatic plantar fasciopathy by studying the changes in pain, mobility and functionality of the lower limb. DESIGN: Pilot, analytical, longitudinal, prospective, controlled, randomized, single-blind study. The sample is of 6 patients from the "Center de Fisioteràpia Teresa Rodríguez", with a clinical diagnosis of plantar fasciopathy and carriers of orthopedic insoles. MAIN MEASUREMENTS: The subjective sensation of pain, the active joint range of the dorsal ankle flexion and the functionality of the lower limb. RESULTS: A statistically significant change in pain evolution was not obtained, despite a difference of 1.67 points on the VAS scale (P <0.296) in the experimental group, compared to the control group. CONCLUSIONS: It can not be confirmed that the structural osteopathic approach reduces the subjective values of pain in patients with symptomatic plantar fasciopathy. In spite of the little statistical significance obtained in this study, higher average values of change have been expressed in the experimental group with respect to the control group in the variables of pain and functionality.
