Comparación de dos intervenciones logopédicas en pacientes bilingües con anomia post-ictus : protocolo de estudio

Abstract

RESUMEN: Introducción: en la afasia, el síntoma mas frecuente y que más persiste en tiempo es la anomia. Más de la mitad de las personas son bilingües por lo que la presencia de pacientes bilingües con anomia post-ictus puede ser alta. La primera lengua que adquirimos se llama lengua materna (L1); cualquier idioma adquirido después de L1 es segunda lengua (L2). Distintos estudios han reportado que los métodos Semantic Feature Analysis (SFA) y Phonological Components Analysis (PCA) son eficaces en el tratamiento de la anomia y ofrecen una generalización lingüística cruzada. Objetivos: determinar cuál de las dos intervenciones es más eficaz para rehabilitar la anomia bilingüe. Averiguar si las terapias son beneficiosas tanto en la lengua tratada como en la no tratada y si mejoran la expresión oral, comunicación y calidad de vida del paciente. Material y métodos: 20 pacientes serán divididos en dos grupos mediante aleatorización. Uno recibirá una intervención semántica (SFA) y otro una fonológica (PCA), ambas planteadas en L2. Se examinarán el lenguaje oral, escrito y la calidad de vida. El análisis de los resultados se hará mediante el test no paramétrico U de Mann Whitney para muestras relacionadas para comparar las puntuaciones intragrupo antes y después del tratamiento; y para muestras independientes parar comparar cuál de las dos terapias es mas eficaz. Discusión/conclusión: mediante las dos terapias, se espera mejorar el acceso al léxico cotidiano del paciente tanto en la lengua tratada como en la no tratada; y así, aumentar su comunicación funcional y calidad de vida.

ABSTRACT: Introduction: In aphasia, the most frequent symptom that persists over time is anomia. More than half of the people are bilingual so the presence of bilingual patients with post-stroke anomia may be high. The first language we acquire is called mother tongue (L1); any language acquired after L1 is second language (L2). Several studies have reported that the Semantic Feature Analysis (SFA) and Phonological Components Analysis (PCA) methods are effective in the treatment of anomia and offer a cross-linguistic generalization. Objectives: to determine which of the two interventions is most effective in rehabilitating bilingual anomie. Find out if therapies are beneficial in both the treated and untreated language and if they improve oral expression, communication and quality of life of the patient. Material and methods: 20 patients will be divided into two groups by randomization. One will receive a semantic intervention (SFA) and another a phonological intervention (PCA), both raised in L2. Oral and written language and quality of life will be examined. The results will be analyzed using Mann Whitney’s nonparametric U test for related samples to compare intragroup scores before and after treatment; and for independent samples to compare which of the two therapies is more effective. Discussion/conclusion: by means of the two therapies, it is expected to improve access to the daily lexicon of the patient in both the treated and the untreated language; and thus, increase their functional communication and quality of life.
