Intervenció fisioterapèutica en pacients amb pneumònia bilateral lleu, moderada i greu post COVID-19

Abstract

RESUM : INTRODUCCIÓ: La condició post COVID-19 es relaciona amb un conjunt de símptomes multiorgànics, que afecta a un nombre creixent de persones. Les severes seqüeles incideixen negativament en la seva qualitat de vida. Un dels sistemes amb més prevalença d’afecció és el respiratori. Estadísticament, fins a un 20% de les persones que sobreviuen a un procés agut, desenvoluparà la condició post COVID-19. OBJECTIUS: Elaborar un pla integral d’intervenció fisioterapèutica en la pneumònia bilateral post COVID-19. RESULTATS: La condició post COVID-19 afecta a qualsevol gènere, raça, edat o estatus sòcio-econòmic. No obstant, el present estudi se centra en tres perfils de pacients, segons la gravetat de la pneumònia que presenten: pneumònia bilateral lleu, moderada i greu post COVID-19. El disseny del pla diagnòstic inclou una anamnesi integral basada en la història clínica i en proves funcionals i diagnòstiques, per tal d’establir el tipus d’intervenció més adient. El pla de tractament, dissenyat també individualment, es basa en tècniques de provada evidència en el millorament respiratori de volums pulmonars, el drenatge de secrecions, l’entrenament de la musculatura respiratòria i l’exercici de força muscular, entre d’altres. El seguiment es realitza, periòdicament, mitjançant una prova de l’estat funcional del/ de la usuari/ària. Tots dos plans es decidiran compartidament amb el/la pacient, respectant la seva opinió i preferències, i, amb la resta de l’equip multidisciplinari. CONCLUSIONS: El pla d’actuació clínica elaborat permet un diagnòstic i tractament específics, i atendre ràpidament i eficient les necessitats dels/ de les pacients amb pneumònia bilateral consegüent a la condició post COVID-19. ABSTRACT : INTRODUCTION: The post-COVID-19 condition corresponds to a set of multi-organ symptoms that affects a large number of patients and generates some sequels, with a negative impact on the quality of life of the patients. One of the systems with the most prevalence of affection is the respiratory system. It is estimated that around 20% of the people who survive an acute process will develop the post-COVID-19 condition. OBJECTS: To make an integral plan of physiotherapist intervention in patients suffering from bilateral pneumonia post COVID-19. RESULTS: The post-COVID-19 condition affects any gender, race, age or socioeconomic status. However, the present study focuses on three profiles of patients, according to the severity of the pneumonia, they present: mild, moderate and severe post-COVID-19 bilateral pneumonia. The design of the diagnostic plan by the Physiotherapy professional includes a comprehensive anamnesis on the clinical history, functional and diagnostic tests, in order to define the most appropriate type of intervention. The treatment plan, designed individually, is based on proven techniques in the respiratory improvement of lung volumes, drainage of secretions, respiratory muscle training and muscle strength exercises, among others. Monitoring is carried out through a test of the user's functional status. Both plans will be decided with the patient, respecting their opinion and preferences, and with the rest of the multidisciplinary team. CONCLUSIONS: The clinical action plan drawn up will allow a specific diagnosis and treatment. It will more quickly and efficiently attend to the needs of patients with bilateral pneumonia resulting from the post-COVID-19 condition.
