Intervenció fisioterapèutica sobre el Sistema Nerviós Autònom en pacients amb Accident Cerebrovascular isquèmic: intervencions i pautes de tractament respecte l’extremitat superior

Abstract

RESUM : INTRODUCCIÓ: L’ictus és una alteració permanent o transitòria del flux sanguini al cervell que succeeix de manera sobtada i que pot causar dèficits a nivell neurològic. És caracteritzada pel desenvolupament de signes focals o globals que comprometen la funció cerebral. Aquesta patologia es coneix a Europa com la segona causa de mort i la primera d’incapacitat en la població adulta. Amb aquesta proposta volem aconseguir la regularització del Sistema Nerviós Autònom i recuperar la funcionalitat de l’extremitat superior afectada posterior a l’accident cerebrovascular. OBJECTIUS: Elaborar un pla d'intervenció i tractament basat en l’evidència actual, el qual relacioni un abordatge en l’alteració del Sistema Nerviós Autònom i en la funcionalitat i simptomatologia de l’extremitat superior en pacients que han patit un accident cerebrovascular. RESULTATS: Per aplicar el pla d’actuació, realitzarem de forma inicial l'anamnesi i exploració/valoració del pacient. A partir dels resultats obtinguts, decidirem si els/les pacients presenten les característiques necessàries per poder iniciar el tractament proposat, ja que d’aquesta manera, es podrà fer un correcte seguiment. El pla d’intervenció es basa en dues fases. La primera fase es centra en la regularització del Sistema Nerviós Autònom. Per una altra banda, la segona fase es centra en la recuperació de l’extremitat superior afectada i el manteniment de la normalització del SNA. CONCLUSIONS: És important una intervenció multidisciplinària, on prèviament a l'aplicació del pla terapèutic, es dugui a terme una valoració específica i detallada basada en l'evidència per poder fer un tractament adaptat i individualitzat. ABSTRACT : INTRODUCTION: A stroke is a permanent or transient disturbance of blood flow to the brain that occurs suddenly and can cause neurological deficits. It is characterised by the development of focal or global signs that compromise brain function. This pathology is known as the second cause of death and the first cause of disability in the adult population. This proposal seeks to achieve regularisation of the Autonomic Nervous System and recover the functionality of the affected upper limb after a stroke. OBJECTIVES: To develop an intervention and treatment plan based on current evidence, which relates the alteration of the Autonomic Nervous System in patients who have suffered a cerebrovascular accident and whose upper limb is affected as a consequence. RESULTS: In order to apply the action plan, we will initially carry out an anamnesis and examination/assessment of the patient. On the basis of the results obtained, we will decide whether the patients have the necessary characteristics to be able to apply the proposed treatment, as this will enable a correct follow-up to be carried out. The intervention plan is based on two phases. The first phase focuses on the regularisation of the Autonomic Nervous System and the second phase focuses on the recovery of the affected upper limb and the maintenance of the normalisation of the SNA. CONCLUSIONS: A multidisciplinary intervention is important, where prior to the application of the therapeutic plan, a specific and detailed evidence-based assessment is carried out in order to be able to make an adapted and individualised treatment.
