Evaluación y abordaje fisioterapéutico del dolor lumbar crónico de origen facetario

Abstract

RESUMEN : INTRODUCCIÓN: El dolor lumbar es una de las principales causas de malestar, baja laboral y uno de los principales motivos de consulta en urgencias, debido a la complejidad de las estructuras de esta zona, dentro de los posibles generadores de dolor, las articulaciones facetarias muestran un gran reto a la hora de diagnosticar y tratar correctamente. OBJETIVOS: Ampliar nuestros conocimientos sobre el dolor lumbar de origen facetario para evaluar a los pacientes con mayor precisión y así ofrecer el tratamiento más eficaz con mayor evidencia. RESULTADOS: Hemos elaborado un programa diagnóstico que permite identificar las facetas como origen del dolor en pacientes que presenten dolor lumbar inespecífico con síntomas compatibles con el dolor facetario. Además, hemos realizado un programa de actuación terapéutica basado en fases de tratamiento con el objetivo de aliviar dos de los principales síntomas que presentan los pacientes facetarios, el dolor y los problemas funcionales CONCLUSIONES: Este plan de actuación permitirá a los profesionales de la fisioterapia realizar un diagnóstico y tratar a los pacientes de manera más adecuada según la evidencia científica existente de mayor relevancia. ABSTRACT : INTRODUCTION: Low back pain is one of the main causes of discomfort, sick leave and one of the main reasons for consultation in the emergency department. Due to the complexity of the structures in this area, within the possible generators of pain, the facet joints show a great challenge when it comes to achieving correct treatment and diagnosis. OBJECTIVES: To expand our knowledge on low back facet pain in order to evaluate patients with greater precision and thus offer the most effective treatment with the best evidence. RESULTS: We have elaborated a diagnostic program that allows us to identify facets as the origin of pain in patients presenting non-specific low back pain with symptoms compatible with facet pain. In addition, we have developed a therapeutic action program based on treatment phases with the objective of alleviating two of the main symptoms presented by facet patients, pain and functional problems. CONCLUSIONS: This action plan will allow physiotherapy professionals to diagnose and treat patients more appropriately according to the most relevant existing scientific evidence.
